【摘要】：目的:總結(jié)踝關(guān)節(jié)骨折CT表現(xiàn)及特點(diǎn),分析CT圖像與常用踝關(guān)節(jié)骨折分型之間的關(guān)系。方法:回顧性調(diào)查369例成人踝關(guān)節(jié)骨折,分析踝穴上1 cm水平橫斷面CT圖像及其特點(diǎn),進(jìn)行踝關(guān)節(jié)骨折X線分型,分析CT圖像與骨折X線分型之間的關(guān)系。結(jié)果:踝穴上1 cm水平橫斷面CT圖像共有8種表現(xiàn)形式,以a、b、c分別指代腓骨骨折、后踝骨折和骨間韌帶(interosseous tibiofibular ligament,IOL)斷裂,369例踝關(guān)節(jié)骨折CT圖像包括:0度(腓骨、后踝、IOL均完整)40例;Ⅰa度(腓骨骨折、后踝完整、IOL完整)60例,Ⅰb度(腓骨完整、后踝骨折、IOL完整)3例,Ⅰc度(腓骨完整、后踝完整、IOL斷裂)26例;Ⅱab度(腓骨骨折、后踝骨折、IOL斷裂)163例,Ⅱac度(腓骨骨折、后踝完整、IOL斷裂)6例,Ⅱbc度(腓骨完整、后踝骨折、IOL斷裂)61例;Ⅲ度(腓骨骨折、后踝骨折、IOL斷裂)10例。根據(jù)Danis-Weber骨折分型:A型骨折18例,B型骨折238例,C型骨折94例,腓骨無骨折19例。分析CT圖像,A、B、C型骨折IOL斷裂的發(fā)生率分別為0、5.9%、88.3%;CT圖像與Danis-Weber分型之間存在相關(guān)性,IOL斷裂的發(fā)生率隨Danis-Weber分型嚴(yán)重程度而改變,經(jīng)秩相關(guān)檢驗(yàn),差異有統(tǒng)計(jì)學(xué)意義(Spearman R=0.781,P0.001)。CT圖像能發(fā)現(xiàn)X線骨折分型無法判斷的IOL斷裂,其發(fā)生率是5.9%。結(jié)論:踝穴上1 cm水平橫斷面CT圖像在術(shù)前能夠清楚判斷下脛腓聯(lián)合IOL損傷情況,與Danis-Weber骨折分型存在良好的相關(guān)性,并能發(fā)現(xiàn)某些X線片中無法判斷的IOL斷裂情況。
[Abstract]:Objective: to summarize the CT findings and characteristics of ankle fractures and analyze the relationship between CT images and classification of ankle fractures. Methods: 369 cases of adult ankle fractures were investigated retrospectively. the CT images and their characteristics of 1 cm horizontal section on ankle acupoints were analyzed. The X-ray classification of ankle fractures was carried out, and the relationship between CT images and X-ray classification of fractures was analyzed. Results: there were 8 manifestations of 1 cm horizontal cross section CT images on ankle acupoints. A, b, c referred to fibular fracture, posterior malleolus fracture and (interosseous tibiofibular ligament,IOL fracture of interosseous ligament, respectively.
