雙源雙能量CT心肌灌注掃描的單能量成像對(duì)線束硬化偽影的校正研究

【摘要】:目的觀察雙能量CT心肌灌注成像中線束硬化偽影(beam-hardening,BH)的好發(fā)部位,比較不同單能量成像對(duì)線束硬化偽影的校正效果和圖像質(zhì)量。方法對(duì)84例需行心臟CT增強(qiáng)檢查的擬診或確診冠心病患者進(jìn)行雙源雙能量CT(100 k Vp/Sn 140 k Vp)心肌灌注掃描,特殊軟件后處理得到心肌碘分布圖和65、75、85、95、105 ke V單能量圖像,以自動(dòng)生成的M圖像(平均加權(quán)混合120 k V)為對(duì)照,觀察線束硬化偽影的發(fā)生部位和數(shù)量,根據(jù)心肌碘含量、CT值、圖像背景噪聲(SD)、信噪比(SNR)、對(duì)比噪聲比(CNR)和圖像質(zhì)量評(píng)分綜合評(píng)價(jià)心肌單能量成像價(jià)值。結(jié)果 65 ke V單能量圖像偽影計(jì)數(shù)為274,隨著能量增加,偽影計(jì)數(shù)逐漸減少(P0.05);左心室的基底段偽影最明顯(36.5%,100/274),其次為間隔壁(25.91%)、上腔靜脈旁段(24.82%)、后壁(12.77%)(P0.05);心肌偽影區(qū)碘含量(-2.37±0.84)mg/g低于非偽影區(qū)碘含量(3.5±0.92)mg/g(t=-19.36,P0.01);85 ke V的非偽影心肌CT值(93.52±18.52)HU與120 k V[(96.06±16.32)HU]最接近,CT值的組間差異有統(tǒng)計(jì)學(xué)意義(P0.01);85 ke V的SD最小(12.98±3.16,P0.01),非偽影心肌的SNR:75 ke V(6.89±1.79)與85 ke V(6.5±1.7)較高,二者無(wú)明顯差別(t=1.5,P=0.14),CNR:75 ke V最高(18.79±6.76,P0.01),85 ke V(15.4±5.67)與65 ke V(15.53±4.56)之間無(wú)明顯差別(t=0.27,P=0.79);圖像質(zhì)量評(píng)分:85 ke V最高,兩名診斷醫(yī)師對(duì)六組圖像進(jìn)行評(píng)分,結(jié)果高度一致(Kappa=0.79,P0.01)。結(jié)論左心室的基底段、間隔壁和上腔靜脈旁易出現(xiàn)線束硬化偽影;正常心肌密度在85 ke V與混合能量120 k V最接近,85 ke V能在最有效糾正線束硬化偽影的同時(shí)保證較好的圖像質(zhì)量。
[Abstract]:Objective to observe the prone position of wire harness sclerosis artifact (beam-hardening,BH) in dual energy CT myocardial perfusion imaging, and to compare the correction effect and image quality of different single energy imaging on wire harness sclerosis artifact. Methods 84 patients with coronary heart disease who needed cardiac CT enhancement were examined by dual source dual energy CT (100 k Vp/Sn 140 k Vp) myocardial perfusion scan). The myocardial iodine distribution map and 65, 75, 95 105 ke V single energy images were obtained by special software post-processing. Compared with the automatically generated M image (average weighted mixed 120 k V), the location and quantity of harden artifacts were observed. according to myocardial iodine content, CT value and background noise, (SD), signal-to-noise ratio (SNR),) was observed. The value of myocardial single energy imaging was evaluated by comparing noise ratio (CNR) with image quality score. Results the artifact count of 65 ke V single energy image was 274. with the increase of energy, the artifact count decreased gradually (P 0.05).
