CT引導(dǎo)下鹽酸溶液化學(xué)消融實(shí)驗(yàn)研究

【摘要】:目的 1、研究6mol/l鹽酸溶液局部注射對新西蘭大白兔機(jī)體的影響,探討6mol/l鹽酸溶液作為化學(xué)消融劑使用的可行性。 2、研究不同劑量鹽酸溶液作用于新西蘭大白兔肝臟、背部肌肉消融范圍的大小,觀察消融后不同階段組織壞死情況及周圍炎性反應(yīng),明確消融區(qū)域組織學(xué)變化,總結(jié)機(jī)體對鹽酸消融區(qū)域修復(fù)規(guī)律。 材料和方法 1、在CT引導(dǎo)下將6mol/l鹽酸溶液用微量泵緩慢注入4只新西蘭大白兔前腿肌肉內(nèi),每只2ml,速度為4ml/h,并在注射前后10min、注射后一周內(nèi)每天取兔耳中動(dòng)脈血化驗(yàn),監(jiān)測肌酐(CR)、肌酸激酶(CK)、天冬氨酸氨基轉(zhuǎn)移酶(AST)、丙氨酸氨基轉(zhuǎn)移酶(ALT)等項(xiàng)目。觀察實(shí)驗(yàn)動(dòng)物基本生命體征,對實(shí)驗(yàn)室檢查結(jié)果進(jìn)行分析。 2、新西蘭大白兔24只,隨機(jī)分為8組,按照計(jì)劃分別將0.5ml、1.0ml、1.5ml、2.0ml鹽酸溶液分步驟以4ml/h的速度滴注入兔背部特定區(qū)域肌肉內(nèi),前4組觀察術(shù)后12h內(nèi)消融區(qū)域的情況;后4組觀察術(shù)后4周內(nèi)鹽酸溶液消融區(qū)域的情況。實(shí)驗(yàn)結(jié)束時(shí)處死動(dòng)物,取材做病理切片,觀察消融范圍及其周圍炎性反應(yīng)和纖維化程度。 2、新西蘭大白兔8只,隨機(jī)分為2組并組內(nèi)編號,第一組肝內(nèi)注射0.5ml鹽酸溶液,第二組肝內(nèi)注射1.0ml鹽酸溶液,分別在注射后1天、1月處死實(shí)驗(yàn)動(dòng)物,取材做病理切片,觀察消融范圍及其周圍炎性反應(yīng)和纖維化程度。 結(jié)果 1.6mol/l鹽酸溶液2ml滴注入兔肌肉組織后,兔外周血實(shí)驗(yàn)室檢查可見肌酐、肌酸激酶、天冬氨酸氨基轉(zhuǎn)移酶、丙氨酸氨基轉(zhuǎn)移酶等指標(biāo)在術(shù)后3天內(nèi)一過性升高,后逐漸恢復(fù)正常。動(dòng)物一般情況良好,,注射局部皮膚潰爛,呈淡黃色。 2.大體病理結(jié)果顯示,0.5ml、1.0ml、1.5ml、2.0ml鹽酸溶液作用于肌肉,消融最大范圍分別為2.77±0.29、3.08±0.75、3.22±0.98、3.46±0.20厘米,約在注射鹽酸溶液12h后出現(xiàn);12h內(nèi)壞死范圍隨鹽酸劑量的遞增、時(shí)間延長而不斷增大,壞死區(qū)域周圍可見粒細(xì)胞、多核巨細(xì)胞聚集;隨時(shí)間推移,12h-4w消融范圍逐漸縮小,3周后壞死組織內(nèi)可見大量的纖維細(xì)胞。 2.8只新西蘭大白兔肝內(nèi)定量注射鹽酸溶液后,基本生命體征平穩(wěn)。定期處死實(shí)驗(yàn)動(dòng)物,取材行病理學(xué)分析。大體病理顯示切面灰白色,質(zhì)地均勻;鏡下可見鹽酸注射區(qū)域壞死明顯,周圍可見炎性反應(yīng)區(qū)域。0.5ml鹽酸溶液作用于肝臟組織,消融24h后,消融范圍約為2.05cm,1月后約為1.72cm;1.0ml鹽酸溶液作用于肝臟組織,消融1月后,消融范圍約為2.75cm,1月后約為2.17cm。 結(jié)論 1、4只新西蘭大白兔經(jīng)6mol/l鹽酸溶液局部注射后,消融效果良好,肌酐、肌酸激酶、天冬氨酸氨基轉(zhuǎn)移酶、丙氨酸氨基轉(zhuǎn)移酶一過性升高,3天后機(jī)體經(jīng)肝臟、腎臟代謝可自行將外周血監(jiān)測指標(biāo)調(diào)整至正常范圍,實(shí)驗(yàn)兔基本生命體征平穩(wěn)。6mol/l鹽酸溶液在CT引導(dǎo)下作為消融劑使用安全可靠。 2、24只新西蘭大白兔肌肉滴注鹽酸溶液后,消融范圍內(nèi)細(xì)胞壞死徹底,消融范圍在12h內(nèi)隨時(shí)間延長而逐漸增大,消融后12小時(shí)至4周,消融范圍隨時(shí)間延長而縮小。范圍與劑量成正比,消融區(qū)域范圍可控,壞死徹底。 3、8只新西蘭大白兔肝臟滴注6mol/l鹽酸溶液后,未見明顯全身反應(yīng),消融局部區(qū)域細(xì)胞核溶解,消融范圍外1cm區(qū)域內(nèi)匯管區(qū)可見壞死,消融范圍可控、細(xì)胞壞死徹底。
