【摘要】：目的:通過(guò)全數(shù)字化乳腺攝影檢發(fā)現(xiàn)鈣化病灶，分析鈣化特點(diǎn)并討論乳腺鈣化的特點(diǎn)對(duì)乳腺X線影像報(bào)告與數(shù)據(jù)系統(tǒng)BIRADSⅡ-Ⅴ類(lèi)診斷的價(jià)值。方法：收集84例患者例行全數(shù)字化乳腺機(jī)攝影檢查發(fā)現(xiàn)93處鈣化病灶，84例患者均為女性患者，年齡在31-76歲之間，平均年齡46.51，她們術(shù)前通過(guò)乳腺攝影檢查，有2位以上的具有豐富乳腺鉬靶診斷經(jīng)驗(yàn)的醫(yī)師，通過(guò)對(duì)鈣化形態(tài)、分布、數(shù)目及伴隨征象分析，得出X線影像報(bào)告與數(shù)據(jù)系統(tǒng)（BIRADS）分類(lèi)報(bào)告系統(tǒng)分類(lèi)結(jié)果。所有患者通過(guò)手術(shù)或穿刺等方法均獲得獲得病理診斷：其中良性病變70例，惡性病變22例，1例為交界性葉狀腫瘤，，通過(guò)分析鈣化的形態(tài)、分布及伴隨征象在不同BIRADS分類(lèi)中的表現(xiàn)以及良惡性比例，探討乳腺鈣化對(duì)BI-RADS分類(lèi)診斷的價(jià)值。結(jié)果:93處鈣化灶良惡性組年齡均數(shù)存在統(tǒng)計(jì)學(xué)差異，良性組70例年齡均數(shù)是45.43歲，惡性組22例年齡均數(shù)是49.95，1例交界性葉狀腫瘤患者年齡54歲。BIRADSⅡ類(lèi)20例，良性19例、惡性1例；Ⅲ類(lèi)26例，良性22例、惡性4例；Ⅳ類(lèi)37例，良性26例、11例；Ⅴ類(lèi)10例，良性3例、惡性6例及1例交界性腫瘤。鈣化分布象限差異存在統(tǒng)計(jì)學(xué)意義。多形性、不均質(zhì)形（顆粒狀）鈣化與其他類(lèi)型鈣化在不同分類(lèi)中存在統(tǒng)計(jì)學(xué)差異，不同鈣化分布在不同分類(lèi)中存在差異。不同分級(jí)中單位面積中鈣化數(shù)目存在統(tǒng)計(jì)學(xué)差異。結(jié)論:通過(guò)乳腺的全數(shù)字化攝影檢查發(fā)現(xiàn)乳腺鈣化灶，分析鈣化的形態(tài)特征、分布情況、單位面積數(shù)目及伴隨征象對(duì)BI-RADS分類(lèi)診斷的價(jià)值。
[Abstract]:Objective: to find calcification lesions by full digital mammography, analyze the characteristics of calcification and discuss the value of calcification in mammography reporting and data system BIRADS class II-V diagnosis. Methods: 93 calcification lesions were detected by digital mammography in 84 patients. 84 patients were female, aged between 31 and 76 years old, with an average age of 46.51. They passed mammography before operation. There are more than two doctors with rich experience in breast mammography diagnosis. Through the analysis of calcification morphology, distribution, number and accompanying signs, the classification results of X-ray image report and data system (BIRADS) classification report system are obtained. All patients were diagnosed pathologically by operation or puncture, including 70 cases of benign lesions, 22 cases of malignant lesions and 1 case of borderline lobular tumor. the morphology of calcification was analyzed. The distribution and accompanying signs in different BIRADS classification and the proportion of benign and malignant, to explore the value of breast calcification in the diagnosis of BI-RADS classification. Results: the age average of benign and malignant calcification was 45.43 years old in benign group, 49.95 years in malignant group and 54 years old in 1 patient with borderline lobular tumor. 19 cases were benign and 1 case was malignant. There were 26 cases of type III, 22 cases of benign, 4 cases of malignant, 37 cases of type IV, 26 cases of benign, 11 cases of benign, 10 cases of class V, 3 cases of benign, 6 cases of malignant and 1 case of borderline tumor. There was significant difference in calcification distribution quadrant. Pleomorphism, heterogeneous calcification and other types of calcification were statistically different in different categories, and the distribution of calcification was different in different categories. There were significant differences in the number of calcification per unit area in different grades. Conclusion: the calcification of breast was found by full digital mammography, and the morphological characteristics, distribution, number per unit area and accompanying signs of calcification were analyzed for the classification and diagnosis of BI-RADS.
