【摘要】：目的對螺旋CT和MRI在隱性骨折早期診斷中的價值進行探討分析。方法將210例可疑隱性骨折患者作為本次研究的對象,行螺旋CT和MRI檢測,比較兩種檢測準確度。結(jié)果本組患者經(jīng)確診,有50例患者為疲勞骨折,34例患者為衰竭骨折,76例為隱性骨內(nèi)骨折,另外50例為隱性創(chuàng)傷骨折。在疲勞骨折患者及隱性骨內(nèi)骨折患者的早期診斷中,MRI診斷確性顯著高于螺旋CT(P0.05);在衰竭骨折患者及隱性創(chuàng)傷骨折患者的早期診斷中,螺旋CT則可以獲得更高的診斷準確率(P0.05)。就敏感度而言,螺旋CT對疲勞骨折、隱性骨內(nèi)骨折、隱性創(chuàng)傷骨折早期診斷方面顯著優(yōu)于MRI(P0.05),而MRI對衰竭骨折早期診斷的特異度高于螺旋CT,螺旋CT對隱性骨內(nèi)骨折的特異度高于MRI(P0.05)。結(jié)論螺旋CT和MRI是兩種非常有效的隱性骨折早期診斷方式,且兩種方式在不同類型的隱性骨折診斷中各具優(yōu)勢,均具有非常重要的臨床價值,臨床在診斷過程中應根據(jù)實際情況采取相應的檢測方式,以提高病情診斷準確率。
[Abstract]:Objective to investigate the value of spiral CT and MRI in the early diagnosis of occult fracture. Methods 210 patients with suspected occult fracture were examined by spiral CT and MRI, and the accuracy of the two methods was compared. Results there were 50 cases of fatigue fracture, 34 cases of failure fracture, 76 cases of recessive intraosseous fracture and 50 cases of recessive traumatic fracture. In the early diagnosis of fatigue fracture and recessive intraosseous fracture, the diagnostic accuracy of MRI was significantly higher than that of spiral CT (P0.05). In the early diagnosis of failure fracture and recessive trauma fracture, spiral CT can obtain higher diagnostic accuracy (P0.05). In terms of sensitivity, spiral CT was significantly superior to MRI in early diagnosis of fatigue fracture, recessive intraosseous fracture and recessive traumatic fracture (P0.05), while MRI was more specific in early diagnosis of failure fracture than spiral CT,. The specificity of spiral CT in the treatment of occult intraosseous fracture was higher than that of MRI (P0.05). Conclusion Spiral CT and MRI are two effective methods for early diagnosis of occult fractures, and the two methods have their own advantages in the diagnosis of different types of occult fractures, and both have very important clinical value. In order to improve the accuracy of the diagnosis, we should adopt the corresponding detection method according to the actual situation in the clinical diagnosis process.
