4D-CT血管造影在急性缺血性腦卒中中的應(yīng)用價值

【摘要】:目的:探討采用256層CT全腦灌注成像重建的4D-CT血管造影在急性缺血性腦卒中中的價值。方法:自2011年12月至2012年9月,共收集52例急性缺血性腦卒中病患的臨床及影像學(xué)資料。所有患者均接受入院CT平掃、256層全腦CTP和頭頸部CTA檢查。CTP動態(tài)掃描獲得全腦的血流灌注信息,通過灌注原始數(shù)據(jù)重建獲得12cm掃描覆蓋范圍內(nèi)的動態(tài)4D-CTA,觀察其檢測大血管管腔狀況的能力。結(jié)果:CTA共檢測11位患者中存在12處顱內(nèi)血管閉塞。4D-CTA顯示顱內(nèi)11處大血管閉塞,敏感性、特異性及陽性預(yù)測值分別為91.7%、100%、100%。結(jié)論:256層CT全腦灌注成像重建的4D-CT血管造影可作為檢測急性缺血性腦卒中大血管閉塞的一種可行方法。
[Abstract]:Objective: to evaluate the value of 4D-CT angiography with 256-slice CT global cerebral perfusion imaging in acute ischemic stroke. Methods: from December 2011 to September 2012, the clinical and imaging data of 52 patients with acute ischemic stroke were collected. All patients received plain CT scan, 256-slice whole brain CTP and head and neck CTA examination. The blood perfusion information of the whole brain was obtained by dynamic CTP scanning, and the dynamic 4D-CTAs covered by 12cm scan were obtained by the reconstruction of the original perfusion data. The ability of detecting the condition of large vascular lumen was observed. Results: there were 12 intracranial vascular occlusions in 11 patients detected by CTA. The sensitivity, specificity and positive predictive value of 4D-CTA were 91.7100% and 100% respectively. Conclusion: the 4D-CT angiography with 256-slice CT whole brain perfusion imaging can be used as a feasible method for the detection of large vascular occlusion in acute ischemic stroke.
