正常成人多層螺旋CT灌注成像的頭皮皮瓣腦血流動力學研究

【摘要】:目的:探討多層螺旋CT灌注成像(MSCTPI)技術在正常人頭皮皮瓣腦血流動力學中的應用。方法:選取近5年因其他原因行全腦灌注及CTA檢查正;颊60例,獲得的數(shù)據(jù)通過體部灌注軟件進行分析,分別測定額部、顳部、枕部頭皮皮瓣的血流量、血容量、達峰時間,并對其進行定量分析。結果:MSCTPI顯示正常成人頭皮皮瓣的平均血流量(TBF)額、顳、枕分別為(47.61±7.06)mL/(min·100g)、(45.52±6.52)mL/(min·100g)、(41.84±6.22)mL/(min·100g);平均血容量(TBV)額、顳、枕分別為(40.43±6.00)mL/100g、(30.13±4.72)mL/100g、(31.89±5.50)mL/100g;達峰時間(TTP)額、顳、枕分別為(25.00±4.02)s、(27.50±4.52)s、(28.00±5.03)s。分別對額部與顳部、額部與枕部的頭皮灌注數(shù)值TBF、TBV、TTP做兩獨立樣本T檢驗,P均0.05,額部的血供較豐富。結論:MSCTPI技術為測量正常成人頭皮血流動力學提供了一種新的方法,為評估頭皮血供的異常界定了一個標尺。為進一步解決大面積顱骨修補術后鈦網(wǎng)外露奠定了基礎。
[Abstract]:Objective: to investigate the application of multislice spiral CT perfusion (MSCTPI) in cerebral hemodynamics of scalp flap in normal subjects. Methods: 60 patients with normal brain perfusion and CTA examination were selected for other reasons in recent 5 years. The data were analyzed by body perfusion software. The blood flow, blood volume and peak time of frontal, temporal and occipital scalp flaps were measured respectively. The quantitative analysis was carried out. Results: MSCTPI showed that the mean blood flow of the scalp flap was (47.61 鹵7.06) mL/ (min 100g), (45.52 鹵6.52) mL/ (min 100g), (41.84 鹵6.22) mL/ (min 100g). The mean blood volume of (TBV), temporal and occipital were (40.43 鹵6.00) mL/100g, (30.13 鹵4.72) mL/100g, (31.89 鹵5.50) mL/100g;, respectively. The peak time of (TTP), temporal and occipital were (25.00 鹵4.02) s, (27.50 鹵4.52) s, (28.00 鹵5.03) s. Two independent T tests were performed on the scalp perfusion values of frontal and temporal, frontal and occipital regions, respectively (P < 0.05). Conclusion: MSCTPI provides a new method for measuring hemodynamics of scalp in normal adults and defines a scale for evaluating the abnormal blood supply of scalp. It lays a foundation for solving the exposure of titanium mesh after large-area skull repair.
