【摘要】：目的探討在道路交通事故致顱腦損傷后精神障礙的傷殘?jiān)u定中精神障礙類型、傷殘等級(jí)及其影響因素方面的差異,為腦外傷所致精神障礙的司法鑒定工作提供有益的參考依據(jù),以便在實(shí)踐中更好地把握規(guī)范。方法對(duì)內(nèi)蒙古自治區(qū)精神衛(wèi)生中心司法鑒定所2014年4月1日至2016年3月30日涉及道路交通事故致顱腦損傷后精神障礙的傷殘?jiān)u定案例在臨床癥狀、精神障礙類型分布、智力損害程度、腦損傷程度、受傷時(shí)間據(jù)鑒定時(shí)間、傷殘等級(jí)等進(jìn)行歸納整理、回顧性分析。結(jié)果共有83例樣本納入本研究,平均評(píng)定時(shí)機(jī)(10.35±6.8)個(gè)月,臨床主要表現(xiàn)為意識(shí)障礙和記憶、語(yǔ)言功能損害。原發(fā)損傷部位較易發(fā)生的部位在額、顳兩葉,不同部位精神損傷類型的比較存在統(tǒng)計(jì)學(xué)意義(χ2=41.83,P0.01),Logistic回歸分析顯示頭顱CT/MRI結(jié)果(r=-0.382)、精神障礙表現(xiàn)(r=-0.476)、智商測(cè)定值(r=-0.358)、原發(fā)損傷程度(r=-0.491)與精神傷殘等級(jí)呈負(fù)相關(guān),均具有統(tǒng)計(jì)學(xué)意義(P0.01)。結(jié)論道路交通事故致顱腦損傷后精神障礙的傷殘?jiān)u定應(yīng)把握恰當(dāng)?shù)蔫b定時(shí)機(jī),原發(fā)損傷程度、精神障礙表現(xiàn)及腦影像學(xué)檢查在評(píng)定中具有重要的地位,智能損害在精神傷殘等級(jí)評(píng)定中應(yīng)慎重。
[Abstract]:Objective to explore the differences in the types of mental disorders, disability grades and their influencing factors in the assessment of mental disorders after craniocerebral injury caused by road traffic accidents, so as to provide a useful reference for the judicial identification of mental disorders caused by brain injuries. In order to better grasp the norms in practice. Methods from April 1, 2014 to March 30, 2016, the Institute of Forensic expertise of Mental Health Center of Inner Mongolia Autonomous region was used to evaluate the clinical symptoms and types of mental disorders in patients with mental disorders after road traffic accidents. The degree of mental impairment, the degree of brain injury, the time of injury according to the time of identification, the grade of disability and so on were summarized and analyzed retrospectively. Results A total of 83 cases were included in this study. The average time of assessment was (10.35 鹵6.8) months. The main clinical manifestations were disturbance of consciousness, memory and language impairment. There was significant difference in the types of mental injury between frontal and temporal lobes (蠂 ~ 2: 41.83), Logistic regression analysis showed that the cranial CT/MRI results were significant (r = -0.382). Mental disorders (r-0.476), intelligence quotient (r-0.358) and primary injury (r-0.491) were negatively correlated with mental disability (P0.01). Conclusion the evaluation of mental disorders after craniocerebral injury caused by road traffic accident should be carried out at the right time, the degree of primary injury, the manifestation of mental disorder and the examination of brain imaging play an important role in the evaluation. Intelligences should be carefully assessed in the rating of mental disability.
