積極性恢復對兔骨腱結(jié)合部過度使用性損傷愈合的影響

【摘要】:過度使用性損傷在運動員中非常多見,運動員長期大強度訓練,使骨腱結(jié)合部經(jīng)常處于過度牽拉狀態(tài),導致?lián)p傷發(fā)生。而此類損傷的治療較為困難,為運動員的訓練比賽都帶來了較大影響。很多運動員在損傷發(fā)生后,未能進行積極休息和治療,繼續(xù)帶傷訓練,反而運動成績下降。因此,本研究旨在觀察運動員在損傷發(fā)生后,應該選擇休息還是繼續(xù)訓練,并如何進行訓練,為對于發(fā)生此類損傷的運動員制定訓練計劃提供理論依據(jù)。 研究對象及方法:研究對象為28只成年雌性新西蘭大白兔(18周齡,體重為2.6±0.2kg),隨機篩選分為積極性恢復組(LL組,n=8),傷后休息組(LR組,n=8),損傷對照組(LC組,n=8)和空白對照組(CON, n=4)四個組別,CON組左右側(cè)后肢均為對照,LL組、LR組和LC組左后肢進行循環(huán)載荷,前四周的電刺激強度均為兔后肢股四頭肌最大收縮力的30%(即7.06±0.71N),每次2小時,每周3次;在此基礎上,LR組休息4周,LL組繼續(xù)以損傷閾下強度(最大強直收縮力的12%,即2.82±0.28N)訓練4周。LR組、LL組和CON組在8周時取材,LC組在4周時取材,經(jīng)過脫鈣、脫水、石蠟包埋、切片、染色等處理,觀察骨腱結(jié)合部組織學變化。 結(jié)果與分析:HE染色顯示,LC組細胞計數(shù)顯著高于CON組,LR組顯著低于LC組,其余組間無差異:纖維軟骨帶厚度,LC組、LR組、LL組均顯著高于CON組,LR組和LL組顯著高于LC組,其余組間無差異;VEGF計數(shù),LC組顯著高于CON組,LR組、LL組顯著低于LC組;其余組間無顯著差異;bFGF計數(shù),LC組、LR組、LL組均顯著高于CON組,LL組顯著高于LR組,其余組間無差異。LC組各項指標的變化,證實了4周造模的成功,LR組和LL組的變化說明休息或是損傷閾下強度訓練都能夠促進損傷的修復。而在bFGF計數(shù)中,LL組的變化提示在還沒有恢復到損傷前,bFGF表達量仍顯著增高,可能是損傷閾下強度的訓練,繼續(xù)刺激了肌腱組織的修復過程。 結(jié)論:通過4周的循環(huán)載荷損傷造模,進行4周的休息或4周的損傷閾下強度訓練均可對骨腱結(jié)合部的損傷有明顯的修復作用;但是4周的休息和閾下強度訓練這兩種方式對組織愈合的影響不同。
[Abstract]:Overuse injury is very common in athletes. The long term intensive training makes the bone-tendon junction often in an overstretched state, leading to injury. The treatment of this kind of injury is more difficult, which has a great impact on the training competition of athletes. After injury, many athletes failed to take active rest and treatment, and continued to train with injuries. Instead, their performance declined. Therefore, the purpose of this study is to observe whether athletes should choose to rest or continue to train after the injury, and how to train, so as to provide a theoretical basis for the athletes who suffer such injuries to make training plans. Methods: Twenty-eight adult female New Zealand white rabbits (18 weeks old, weight 2.6 鹵0.2kg) were randomly divided into two groups: positive recovery group (LL group, nong8) and post-injury rest group (LR group, nong8 group). The left and right hind limbs in the CON group were all controlled, and the left hind limbs in the LL group, LR group and LC group were subjected to cyclic loading, respectively, in the four groups of injured control group (LC group, nong8 group) and blank control group (CON, nong4 group). The intensity of electrical stimulation in the first four weeks was 30% of the maximal contractility of the quadriceps femoris muscle of the hind limb (i.e. 7.06 鹵0.71 N), 2 hours each time, 3 times a week. On this basis, the LR group rested for 4 weeks, the LL group continued to train for 4 weeks with the subthreshold intensity of injury (the maximum contractile force of rigidity, i.e. 2.82 鹵0.28N). The LR group, LL group and CON group took the material at 8 weeks, and the LC group took the material at 4 weeks after decalcification. Dehydration, paraffin embedding, section and staining were used to observe the histological changes of bone tendon junction. Results and analysis: HE staining showed that the cell count of LC group was significantly higher than that of CON group, and that of LR group was significantly lower than that of LC group. There was no difference between other groups: the thickness of fibrochondrocytes, LC group, LR group and LL group were significantly higher than that of CON group. LR group and LL group were significantly higher than LC group, the other groups had no difference. VEGF counts in LC group were significantly higher than those in CON group, LR group and LL group were significantly lower than those in LC group, but there was no significant difference between other groups. BFGF count, LC group, LR group, LL group were significantly higher than CON group, LL group was significantly higher than LR group, and there was no difference between other groups. Changes in LR group and LL group showed that rest or subthreshold intensity training could promote the repair of injury. In the bFGF count, the changes in LL group indicated that the expression of bFGF was still significantly increased before the injury was recovered, which may be due to the training of subthreshold intensity of injury, which continued to stimulate the repair process of tendon tissue. Conclusion: after 4 weeks of cyclic load injury model, 4 weeks rest or 4 weeks of subthreshold strength training can significantly repair the injury of bone tendon junction. But four weeks of rest and subliminal intensity training had different effects on tissue healing.
