【摘要】：目的探討盆腔腫瘤患者放療前進(jìn)行膀胱充盈訓(xùn)練的可靠性。方法選取40例盆腔腫瘤患者,根據(jù)是否有盆腔手術(shù)史分為手術(shù)組19例和無手術(shù)組21例,兩組均在放療前進(jìn)行膀胱充盈訓(xùn)練,確保患者排空膀胱后每次飲水量一致,采用膀胱容量測量儀進(jìn)行測量并記錄,同時(shí)完成尿感量表評分。對比分析兩組患者膀胱充盈訓(xùn)練的預(yù)測值與測量值,Pearson相關(guān)性分析尿感與測量值的相關(guān)性,并評價(jià)放療過程中膀胱容量預(yù)測值與測量值的變化情況。結(jié)果兩組預(yù)測值與測量值比較差異均無統(tǒng)計(jì)學(xué)意義(P0.05),無手術(shù)組患者的預(yù)測值及測量值均顯著高于手術(shù)組(P0.01);無手術(shù)組患者尿感與測量值的相關(guān)性(r=0.817,P=0.007)比手術(shù)組更高(r=0.734,P=0.024);兩組隨著膀胱充盈訓(xùn)練的進(jìn)展預(yù)測值及測量值均明顯上升(P0.05);放療過程中患者的膀胱容量預(yù)測值與測量值總體趨勢一致,均隨著放療進(jìn)展逐漸下降,且患者的預(yù)測值會(huì)略高于測量值。結(jié)論膀胱充盈訓(xùn)練能夠幫助患者獲得可靠穩(wěn)定的尿感,即使存在盆腔手術(shù)史的患者也同樣可以接受膀胱充盈訓(xùn)練,在放療中患者的膀胱充盈量會(huì)隨著放療進(jìn)展而逐漸下降,尿感可靠性也有所降低,因此在放療后期建議采用膀胱容量探測儀進(jìn)行膀胱充盈量的檢測。
[Abstract]:Objective to evaluate the reliability of bladder filling training in pelvic tumor patients before radiotherapy. Methods 40 patients with pelvic neoplasms were divided into two groups according to the history of pelvic surgery: 19 cases in the operation group and 21 cases in the non-operative group. The two groups were given bladder filling training before radiotherapy to ensure the same amount of water consumed each time after the bladder emptying. The bladder volume meter was used to measure and record, and the urinary sensory scale score was completed at the same time. The predictive value and measurement value of bladder filling training in two groups were compared and compared. The correlation between urinary sensation and measurement value was analyzed by Pearson correlation and the changes of bladder volume predictive value and measurement value during radiotherapy were evaluated. Results there was no significant difference between the predicted values and the measured values of the two groups (P0.05). The predicted values and the measured values of the patients in the non-operation group were significantly higher than those in the operation group (P0.01). The correlation between urinary sensation and measurement value (r = 0.817) was higher in the non-operative group than that in the operation group (r = 0.734), and the predictive value and the measured value were significantly increased with the development of bladder filling training in both groups (P0.05). The predicted value of bladder volume was consistent with the measured value in the course of radiotherapy. The predicted value of bladder volume decreased with the progress of radiotherapy, and the predicted value of the patient was slightly higher than the measured value. Conclusion bladder filling training can help patients to obtain a reliable and stable sense of urine, even if there is a history of pelvic surgery patients can also receive bladder filling training, the bladder filling volume of patients with radiotherapy will gradually decrease with the progress of radiotherapy. The reliability of urinary sensitivity is also reduced, so it is recommended that bladder volume detector be used to detect bladder filling in late radiotherapy.
