【摘要】：目的:探討MDCT靶掃描技術(shù)對胃癌術(shù)前分期的應(yīng)用價值，以AJCC/UICC第七版TNM分期手冊為參照標(biāo)準(zhǔn)。 材料與方法:收集41例行胃癌多排螺旋常規(guī)兩期掃描+動脈后期CT靶掃描、并取得手術(shù)病理結(jié)果的病例。由2名有經(jīng)驗的放射醫(yī)師依據(jù)第七版AJCC/UICC腫瘤TNM分期標(biāo)準(zhǔn)評價并記錄胃癌CT“T”分期、胃癌部位、胃癌區(qū)胃壁厚度、胃癌病理類型對CT“T”分期是否有影響。分析結(jié)果與病理對照，并運用統(tǒng)計學(xué)方法對比常規(guī)兩期增強組及常規(guī)增強+動脈后期靶掃描組兩組方法診斷胃癌的敏感性、特異性、準(zhǔn)確性。 結(jié)果:靶掃描組評價胃癌敏感性、特異性、準(zhǔn)確性均優(yōu)于常規(guī)兩期增強組。靶掃描組的T分期總準(zhǔn)確率為85.3%、兩期增強組58.5%。靶掃描組早期胃癌T1a的敏感率為60%、T1b的敏感率為75%。 結(jié)論:胃靶掃描技術(shù)對胃癌T分期的診斷與病理診斷的符合率高，其診斷定性的準(zhǔn)確性及病變被檢出的敏感性均優(yōu)于傳統(tǒng)兩期增強掃描。
[Abstract]:Objective: to evaluate the value of MDCT target scanning in preoperative staging of gastric cancer, using the seventh edition of AJCC/UICC TNM staging manual as the reference standard. Materials and methods: 41 patients with gastric cancer underwent multislice spiral conventional two-phase arterial CT target scanning at the late stage of arterial scanning and obtained surgical and pathological results. According to the seventh edition AJCC/UICC tumor TNM staging criteria, two experienced radiologists evaluated and recorded the CT "T" staging of gastric cancer, the location of gastric cancer, the thickness of gastric wall, and the pathological type of gastric cancer. The results were compared with pathological findings. The sensitivity specificity and accuracy of the two methods in diagnosing gastric cancer were compared with those in the conventional two-phase enhancement group and the conventional contrast-enhanced arterial target scanning group. Results: the sensitivity, specificity and accuracy of target scan group in evaluating gastric cancer were better than that of conventional two-phase enhancement group. The total accuracy of T staging was 85.3 in the target scan group and 58.5 in the two phase enhancement group. The sensitivity rate of T 1a in target scan group was 60 and 75 in target scan group. Conclusion: the diagnostic accuracy and sensitivity of gastric target scan for T staging and pathological diagnosis of gastric cancer are higher than those of conventional two-phase enhanced scan.
