基于CTA和眼底成像觀察頸內(nèi)動(dòng)脈狹窄程度與視網(wǎng)膜血管管徑的關(guān)系

【摘要】:目的分析頸內(nèi)動(dòng)脈(ICA)狹窄程度與同側(cè)視網(wǎng)膜血管管徑的關(guān)系,探討通過觀察視網(wǎng)膜血管管徑改變?cè)u(píng)估ICA狹窄的準(zhǔn)確性及可行性。方法基于CTA和眼底成像技術(shù),分析243例患者單側(cè)ICA狹窄程度和同側(cè)視網(wǎng)膜血管管徑資料,根據(jù)ICA最大狹窄率(Rmax)將患者分為無狹窄、輕度狹窄、中度狹窄和重度狹窄共4組,分析視網(wǎng)膜血管管徑的組間差異及其與ICA狹窄程度的相關(guān)性。結(jié)果中、重度狹窄組的視網(wǎng)膜中央靜脈管徑(CRVE)較無狹窄和輕度狹窄組明顯增寬(P均0.05),4組間視網(wǎng)膜中央動(dòng)脈管徑(CRAE)和動(dòng)靜脈管徑比值(AVR)差異無統(tǒng)計(jì)學(xué)意義(P均0.05)。Rmax與同側(cè)CRVE呈正相關(guān)(r=0.27,P0.01),與AVR呈負(fù)相關(guān)(r=-0.16,P0.05),與CRAE無相關(guān)性(P0.05)。CRVE是同側(cè)Rmax的影響因素(B=0.243,P0.01),但模型的調(diào)整R2較小(0.173)。以229.5μm為最佳臨界值,CRVE診斷ICA中重度狹窄的ROC曲線下面積為0.619,敏感度和特異度為80.3%、40.1%。結(jié)論 CRVE對(duì)ICA狹窄程度的評(píng)估與預(yù)測(cè)具有一定價(jià)值。
[Abstract]:Objective to analyze the relationship between the degree of (ICA) stenosis of internal carotid artery and the diameter of ipsilateral retinal vessels, and to explore the accuracy and feasibility of evaluating ICA stenosis by observing the changes of retinal vascular diameter. Methods based on CTA and fundus imaging, the data of unilateral ICA stenosis and ipsilateral retinal vascular diameter in 243 patients were analyzed. According to the maximum stenosis rate of ICA, the patients were divided into 4 groups: no stenosis, mild stenosis, moderate stenosis and severe stenosis. To analyze the difference of retinal vascular diameter and its correlation with the degree of ICA stenosis. As a result, The diameter of central retinal vein (CRVE) in severe stenosis group was significantly wider than that in non-stenosis group and mild stenosis group (P0. 05). There was no significant difference in (CRAE) and (AVR) ratio between 4 groups (P 0. 05). Rmax vs ipsilateral CRVE). Positive correlation (r = 0.27, P 0.01), negative correlation with AVR (r = 0.16, P 0.05), no correlation with CRAE (P0.05,). CRVE was an influential factor of ipsilateral Rmax (P 0.05, P 0.01), but the adjusted R2 of the model was smaller (0.173). With 229.5 渭 m as the best critical value, the area under the ROC curve of CRVE for the diagnosis of moderate and severe stenosis of ICA was 0.619, and the sensitivity and specificity were 80.3% and 40.1% respectively. Conclusion CRVE is valuable in the evaluation and prediction of ICA stenosis.
