不同頻率振動(dòng)刺激對(duì)廢用性肌萎縮大鼠康復(fù)效果及機(jī)制研究

【摘要】:骨骼肌廢用性萎縮是臨床醫(yī)學(xué)、康復(fù)醫(yī)學(xué)的重要研究課題之一。本文研究骨骼肌廢用性萎縮的康復(fù)過(guò)程中應(yīng)用振動(dòng)刺激進(jìn)行干預(yù)的效果及機(jī)制。 研究目的:以動(dòng)物實(shí)驗(yàn)為基礎(chǔ),從形態(tài)學(xué)和分子生物學(xué)的角度進(jìn)行觀察,比較安全范圍內(nèi)不同頻率振動(dòng)刺激對(duì)骨骼肌廢用性萎縮的康復(fù)效果,并對(duì)其作用機(jī)制進(jìn)行初步探討,為振動(dòng)訓(xùn)練在廢用性肌萎縮康復(fù)中的應(yīng)用提供參考依據(jù)。 研究方法:選用雌性SD大鼠50只隨機(jī)分為對(duì)照組(A)、低頻振動(dòng)組(B)、高頻振動(dòng)組(C)、懸吊組(D)、自然恢復(fù)組(E)等5組。懸吊14天后,取D組大鼠比目魚肌肌肉組織留置標(biāo)本,分別給予另外兩組以低頻20Hz和高頻30Hz的振動(dòng)刺激訓(xùn)練。對(duì)照組和自然恢復(fù)組保持飲食、活動(dòng)不變。4周后取4組大鼠比目魚肌肌肉組織為標(biāo)本,比較各組濕重體重比;并對(duì)大鼠比目魚、瘛ⅱ蛐图±w維比例、平均橫截面積(CSA)進(jìn)行定量分析;采用western blotting印跡法對(duì)IGF-1、MyoD、Myogenin、Myostatin、TGF-β、bFGF等幾種肌肉生長(zhǎng)因子蛋白進(jìn)行定量分析并進(jìn)行比較。探討各種肌肉生長(zhǎng)因子在振動(dòng)刺激作用下的變化及其對(duì)肌萎縮的影響。 研究結(jié)論: (1)低頻和高頻的振動(dòng)刺激均能夠有效促進(jìn)廢用性肌萎縮模型大鼠比目魚肌的形態(tài)結(jié)構(gòu)的恢復(fù):其中肌肉濕重增加、Ⅰ型和Ⅱ型肌纖維平均橫截面積的增加,還能夠有效對(duì)抗肌纖維類型由Ⅰ型向Ⅱ型的轉(zhuǎn)化。 (2)低頻和高頻的振動(dòng)刺激均能夠使廢用性肌萎縮模型大鼠比目魚肌的IGF-I、MyoD等產(chǎn)生過(guò)量的表達(dá)。而Myostatin的表達(dá)減少,從促進(jìn)骨骼肌蛋白合成、抑制骨骼肌蛋白分解的方面促進(jìn)了骨骼肌萎縮的恢復(fù)。 (3)不同頻率的振動(dòng)刺激康復(fù)效果有一定的差別,綜合形態(tài)學(xué)和骨骼肌蛋白因子表達(dá)測(cè)定結(jié)果分析,其中20Hz的低頻振動(dòng)刺激對(duì)大鼠萎縮肌康復(fù)效果更明顯。 (4)不同頻率振動(dòng)刺激作用下的肌肉生長(zhǎng)調(diào)節(jié)因子中,Myogenin與MyoD, TGF-β與Myostatin雖屬于同一家族,但并未表現(xiàn)一致性。 研究建議: (1)適宜頻率的振動(dòng)刺激作為一種有效的康復(fù)干預(yù)于段,可以應(yīng)用于廢用性肌萎縮康復(fù)治療。(2)對(duì)于骨骼肌廢用性萎縮的轉(zhuǎn)歸和干預(yù)方面的觀察,根據(jù)本文研究結(jié)果,IGF-1、MyoD和Mvostatin可以作為比較理想的分了生物學(xué)指標(biāo)來(lái)應(yīng)用。
[Abstract]:Skeletal muscle disuse atrophy is one of the important research topics in clinical medicine and rehabilitation medicine. This paper studies the effect and mechanism of vibration stimulation in the rehabilitation of skeletal muscle disuse atrophy. Objective: to compare the effects of different frequency vibration stimuli on skeletal muscle disuse atrophy in the safe range, based on animal experiments, and to explore the mechanism of the effects of vibration stimulation on skeletal muscle disuse atrophy in terms of morphology and molecular biology.
