低劑量CT掃描檢測(cè)肺結(jié)節(jié)的實(shí)驗(yàn)研究

【摘要】:目的評(píng)價(jià)能譜CT胸部低劑量掃描的圖像質(zhì)量和對(duì)肺結(jié)節(jié)檢出的敏感性,探討最佳的掃描參數(shù)。 方法制作三組不同大小的軟組織密度的小結(jié)節(jié)(直徑分別為10mm、5mm、3mm),置入充氣膨脹的豬肺中,使用能譜CT分別行常規(guī)劑量掃描(管電壓120kVp,螺距0.984:1,管電流150mA)與低劑量掃描(管電壓120kVp,螺距0.984:1,管電流50mA、30mA、20mA、10mA),記錄不同掃描方式的輻射劑量,評(píng)價(jià)圖像的噪聲與偽影情況,評(píng)估低劑量掃描對(duì)肺結(jié)節(jié)檢出的敏感性。 結(jié)果本研究胸部低劑量掃描的輻射劑量?jī)H為常規(guī)劑量的6.7%—33.3%。低劑量掃描測(cè)得圖像的CT值與常規(guī)劑量相比無(wú)明顯差異(P0.05),但是CT值標(biāo)準(zhǔn)差的差異有統(tǒng)計(jì)學(xué)意義(P0.05)。僅在20mA和lOmA掃描的圖像中出現(xiàn)條形偽影。50mA掃描與常規(guī)劑量掃描一致性較好(0.4≤kappa0.75);20mA掃描能清晰顯示肺支氣管Ⅲ級(jí)以上分支,并能檢出所有結(jié)節(jié);lOmA掃描會(huì)遺漏直徑3mm的結(jié)節(jié)。 結(jié)論能譜CT胸部低劑量掃描的輻射劑量明顯降低;降低管電流雖然使圖像噪聲和偽影增大,但是仍然可以較好的顯示肺內(nèi)結(jié)構(gòu)和病灶;實(shí)驗(yàn)條件下20mA可作為檢測(cè)肺結(jié)節(jié)的可選劑量。
[Abstract]:Objective to evaluate the image quality of low dose CT chest scanning and its sensitivity to the detection of pulmonary nodules and to explore the best scanning parameters. Methods three groups of small nodules (10 mm to 5 mm / 3 mm in diameter) with different sizes of soft tissue densities were made and placed into inflatable pig lungs. Energy dispersive CT was used for routine dose scanning (tube voltage 120kVp, pitch 0.984: 1, tube current 150mA) and low-dose scanning (tube voltage 120kVp, pitch 0.984: 1, tube current 50mAn 30mA10 Ma). Radiation doses of different scanning methods were recorded, and the noise and artifacts of the images were evaluated. To evaluate the sensitivity of low dose scanning to the detection of pulmonary nodules. Results the radiation dose of low-dose chest scan was only 6.7-33. 3% of the conventional dose. There was no significant difference in CT value between low dose scan and conventional dose (P0.05), but the difference of standard deviation of CT value was statistically significant (P0.05).
