【摘要】：目的探討Singh指數(shù)在老年女性髖部脆性骨折患者骨密度評估中的應用價值。方法將216例老年女性髖部脆性骨折患者按年齡分為4組,60~70歲組53例,71~80歲組83例,81~90歲組68例,90歲組12例。行髖部X線攝影,評估患者健側(cè)Singh指數(shù),并將Singh指數(shù)≤Ⅳ級作為骨質(zhì)疏松診斷標準。分析2名診斷醫(yī)師間隔1個月前后兩次評估Singh指數(shù)分級的一致性;比較各年齡組患者身高、體質(zhì)量及體質(zhì)量指數(shù)(BMI)及Singh指數(shù)的差異;并評價髖部Singh指數(shù)與患者年齡、BMI的相關性。結(jié)果 2名醫(yī)師評估Singh指數(shù)分級的觀察者內(nèi)Kappa值分別為0.631、0.615,觀察者間Kappa值分別為0.622、0.598。應用Singh指數(shù)診斷骨質(zhì)疏松的總體檢出率為89.35%(193/216)。各年齡組患者身高、體質(zhì)量及BMI差異均無統(tǒng)計學意義(P均0.05),Singh指數(shù)差異有統(tǒng)計學意義(H=56.077,P0.001)。Singh指數(shù)與患者年齡呈負相關(r=-0.656,P0.001),與BMI無相關性(r=-0.008,P=0.902)。結(jié)論 X線平片Singh指數(shù)雖然相對經(jīng)濟、簡便且較為實用,但重復性較低,不能完全反映老年女性脆性骨折患者骨密度情況。
[Abstract]:Objective to evaluate the value of Singh index in the evaluation of bone mineral density (BMD) in elderly women with fragile hip fractures.
