雙源CT多時相前瞻性冠狀動脈成像應(yīng)用于竇性心動過速患者的探討

【摘要】:目的探討多時相前瞻性冠狀動脈CTA顯示竇性心動過速患者各支冠狀動脈的最佳時相,以提高前瞻性冠狀動脈CTA在竇性心動過速患者中的應(yīng)用價值。方法應(yīng)用Flex Padding技術(shù)對46例臨床懷疑冠心病的竇性心動過速患者行多時相前瞻性冠狀動脈CTA檢查。數(shù)據(jù)采集中心時相設(shè)為40%,以相對時相40%重組圖像后,在180~320 ms間期內(nèi)再以10 ms的間隔重組15個絕對時相。選擇顯示各支冠狀動脈的最佳時相并進行后處理。分析直徑≥1.5 mm的冠狀動脈節(jié)段的圖像質(zhì)量(1~4分),比較單時相和多時相重組的圖像質(zhì)量,并記錄輻射劑量。結(jié)果 46例竇性心動過速患者的右冠狀動脈(RCA)、左前降支(LAD)、左回旋支(LCX)最佳重組時相不一致。71.7%的RCA最佳重組時相位于280~300 ms;52.2%的LAD最佳重組時相位于210~230 ms,82.6%的LAD最佳重組時相位于200~250 ms;63.1%的LCX最佳重組時相位于240~260 ms。40%的單時相重組圖像質(zhì)量評分平均為2.81±0.62,合格率為57.6%;經(jīng)過多時相重組后,圖像質(zhì)量評分平均達到3.57±0.55,合格圖像達到92.6%。單時相與多時相重組的RCA、LAD、LCX圖像質(zhì)量評分存在統(tǒng)計學差異(P0.05)。平均有效輻射劑量(ED)為(2.46±0.55)m Sv。結(jié)論竇性心動過速患者RCA、LAD、LCX的最佳重組時相不一定相同,單時相重組難以獲得滿意的圖像,多時相重組可以提高竇性心動過速患者前瞻性冠狀動脈CTA的圖像質(zhì)量,同時顯著降低輻射劑量。
