吸純氧和高度提升對低壓艙耳氣壓功能檢查的復(fù)合影響

【摘要】:目的比較低壓艙內(nèi)4000m吸空氣和6900m吸純氧對耳氣壓功能的影響。方法 11名男性健康志愿者采用自身對照方法,均參加空氣組(4000m暴露呼吸空氣)和純氧組(6900m暴露呼吸純氧)兩組試驗,兩次試驗間隔2周以上。志愿者在地面高度分別吸空氣或純氧1h,然后在低壓艙內(nèi)以20m/s的速度上升至目標(biāo)高度停留5min,再以20m/s的速度下降至地面高度。記錄兩組志愿者試驗前后不同時間點主觀癥狀、鼓膜充血程度、聲導(dǎo)抗和純音聽閾等指標(biāo)的變化,并對聲導(dǎo)抗指標(biāo)和聽閾變化進行統(tǒng)計學(xué)分析。結(jié)果檢查過程中空氣組和純氧組分別有4人(4耳)和7人(7耳)主訴發(fā)生耳痛,且純氧組發(fā)生壓痛的高度較高。出艙即刻兩組分別有3人(3耳)和5人(5耳)鼓膜Ⅲ度充血,試驗后純氧組有4人(6耳)充血程度隨時間延長而加重,出艙3h及24h兩組鼓膜充血程度差異有統(tǒng)計學(xué)意義。出艙即刻兩組分別有11人(15耳)和11人(14耳)為正常的A型鼓室圖。純氧組出艙即刻靜態(tài)聲順值的增加量明顯高于空氣組(P0.05),出艙3h和24h中耳壓的減少量明顯高于空氣組(P0.05)。兩組試驗均可致咽鼓管功能下降。出艙3h和24h時純氧組個別頻率純音聽閾的增加量明顯高于空氣組(P0.05)。結(jié)論吸入純氧并增加高度可提高低壓艙耳氣壓功能檢查的篩選度,且對24h內(nèi)的鼓膜充血程度、聲導(dǎo)抗和純音聽閾有較大影響。
[Abstract]:Objective to compare the effects of 4000m inhalation of air and 6900m of pure oxygen on ear pressure in low pressure chamber. Methods 11 healthy male volunteers took part in the air exposure test (4000m exposure) and the pure oxygen group (6900m exposure breathing pure oxygen) with the interval of more than 2 weeks. The volunteers inhaled air or pure oxygen at the ground height for 1 hour, then rose to the target height at the speed of 20m/s for 5 min, then dropped to the ground level at the speed of 20m/s. The changes of subjective symptoms, tympanic membrane hyperemia, acoustic conductance and pure tone hearing threshold were recorded at different time points before and after the experiment, and the changes of acoustic impedance index and hearing threshold were analyzed statistically. Results there were 4 cases (4 ears) and 7 cases (7 ears) of ear pain in air group and pure oxygen group respectively, and the degree of tenderness in pure oxygen group was higher.
