【摘要】：目的精確定位大腿肌內(nèi)側(cè)群痙攣肌外神經(jīng)阻滯靶點(diǎn)的位置。方法設(shè)計(jì)經(jīng)恥骨結(jié)節(jié)至股骨大轉(zhuǎn)子外側(cè)皮膚的連線為橫向的H參考線,經(jīng)恥骨結(jié)節(jié)至股骨內(nèi)上髁水平的連線為縱向的L參考線。解剖暴露經(jīng)甲醛固定的10具成人尸體的20側(cè)閉孔神經(jīng)及其肌支,涂抹硫酸鋇于它們表面,烘干,X線攝片,PACS軟件測(cè)量閉孔神經(jīng)肌支肌外阻滯靶點(diǎn)在H線和L線上的百分位置。結(jié)果閉孔神經(jīng)股薄肌支、長(zhǎng)收肌支、短收肌支以及大收肌支在肌外的近側(cè)運(yùn)動(dòng)點(diǎn)(P點(diǎn))分別位于距恥骨結(jié)節(jié)外側(cè)的大腿寬度(H線)的(21.48±1.80)%、(25.85±1.23)%、(28.07±1.65)%和(29.18±2.07)%處,位于距恥骨結(jié)節(jié)遠(yuǎn)側(cè)的大腿長(zhǎng)度(L線)的(8.83±1.01)%、(8.83±1.01)%、(7.57±0.63)%和(7.57±0.63)%處;遠(yuǎn)側(cè)運(yùn)動(dòng)點(diǎn)(D點(diǎn))分別位于距恥骨結(jié)節(jié)外側(cè)大腿寬度(H線)的(16.9±1.33)%、(27.70±2.15)%、(31.18±2.18)%和(35.78±2.79)%處,位于距恥骨結(jié)節(jié)遠(yuǎn)側(cè)大腿長(zhǎng)度(L線)的(35.57±2.77)%、(26.9±1.96)%、(24.26±1.91)%和(28.04±2.17)%處。結(jié)論這些靶點(diǎn)的準(zhǔn)確定位可提高大腿肌內(nèi)側(cè)群痙攣肌外神經(jīng)靶點(diǎn)阻滯的安全性和有效性。
[Abstract]:Objective to locate precisely the position of the target of extramuscular nerve block in the medial group of thigh muscle. Methods the line from the pubic tubercle to the lateral skin of the greater trochanter of the femur was designed as the transverse H reference line, and the line from the pubic tubercle to the level of the medial epicondyle of the femur was designed as the longitudinal L reference line. Twenty obturator nerves and their muscular branches of 10 adult cadavers fixed by formaldehyde were dissected and barium sulfate was applied on their surfaces. The PACS software was used to measure the percentage of the external blocking targets of muscle branches of obturator nerve on H and L lines.
