血管內(nèi)皮細(xì)胞損傷與放射性肺損傷的相關(guān)實(shí)驗(yàn)研究

【摘要】:目的:探討大鼠肺組織受照射后,血管內(nèi)皮細(xì)胞損傷導(dǎo)致放射性肺損傷發(fā)生、發(fā)展的可能致病機(jī)理。 方法:本研究選用60Coγ射線治療機(jī)單次照射大鼠右肺,照射劑量分別為OGy、7.OGy、14.4Gy,建立大鼠放射性肺損傷模型,選擇照射后1天(以下以d表示)、7d、30d、90d和180d為觀察時(shí)間點(diǎn),取對(duì)照組及各照射組大鼠的肺組織進(jìn)行相關(guān)檢測(cè),包括:①大鼠肺放射性損傷的病理學(xué)觀察,包括肉眼觀察、H-E染色及masson染色,光學(xué)顯微鏡觀察;②免疫組織化學(xué)方法檢測(cè)肺組織CD34、CD105、VEGF和FⅧ:RAg的表達(dá)水平。 結(jié)果:(1)肉眼觀察:對(duì)照組大鼠肺組織淡紅色,表面光滑,彈性好,7.0Gy組未見明顯的出血、水腫及纖維化改變;14.4Gy組早期可見片狀出血、水腫和腫脹,照射后90d,肺組織暗紅色,肺組織表面有白色斑點(diǎn)或斑片狀,照射后180d,大部分肺組織表面呈灰白色,肺體積縮小,肺表面凹凸不平,彈性明顯降低、質(zhì)地較硬,部分肺葉塌陷。(2)光鏡下病理組織學(xué)所見:對(duì)照組肺組織結(jié)構(gòu)正常;7.0Gy組照射后早期(1d、7d),肺泡腔及間隔可略見輕微滲出、充血和水腫;14.4Gy組肺泡腔及間隔可見滲出、充血、水腫和炎性細(xì)胞浸潤(rùn),局部可見紅細(xì)胞滲出;照射后30d,7.0Gy組肺泡腔及間隔出現(xiàn)輕微水腫和炎性細(xì)胞浸潤(rùn);14.4Gy組見肺泡間隔增寬、水腫、炎性細(xì)胞浸潤(rùn)和出血,以及肺泡間隔及部分肺泡腔內(nèi)纖維素樣的滲出物,并且可見膠原形成;照射后90d,14.4Gy組肺泡間隔進(jìn)一步增寬,局部仍見出血,部分肺泡腔塌陷、不張、肺大泡形成并且出現(xiàn)膠原形成;照射后180d,14.4Gy組出現(xiàn)明顯的膠原形成和纖維化改變。(3)免疫組織化學(xué)檢測(cè):①CD34,與正常對(duì)照組比較,14.4Gy組除1d外,其余各觀察時(shí)點(diǎn)均有顯著差異(P0.05),而7.0Gy組各時(shí)間點(diǎn)均無(wú)顯著性差異(P0.05);②CD105,與正常對(duì)照組比較,7.0Gy組各時(shí)點(diǎn)均無(wú)顯著差異(P0.05);14.4Gy組除1d,90d外,其余各觀察時(shí)點(diǎn)均有顯著差異(P0.05);③VEGF,與正常對(duì)照組比較,7.0Gy組無(wú)顯著差異(P0.05);14.4Gy組,除90d、180d有顯著差異(P0.05)外,其余時(shí)點(diǎn)均無(wú)顯著差異(P0.05);④FVⅢ:RAg,與正常對(duì)照組比較,14.4Gy組從1d開始,各時(shí)點(diǎn)均有顯著差異(P0.05),而7.0Gy組各時(shí)間點(diǎn)均無(wú)顯著性差異(P0.05)。 結(jié)論:肺組織放射損傷后,微循環(huán)中成熟內(nèi)皮細(xì)胞數(shù)量的進(jìn)行性減少,是新生毛細(xì)血管生成障礙,纖維性修復(fù)慢性遷延的根本原因。
