【摘要】：目的應(yīng)用64層螺旋CT門靜脈期成像,分析胰源性門靜脈高壓側(cè)支循環(huán)的特征。資料與方法回顧性分析2013年8月-2015年5月于西南醫(yī)科大學(xué)附屬醫(yī)院診斷并經(jīng)病理證實的52例胰源性門靜脈高壓患者的CT門靜脈期圖像,觀察其側(cè)支循環(huán)情況。結(jié)果 52例患者中孤立性脾靜脈阻塞(ISVO)21例,非孤立性脾靜脈阻塞(NISVO)31例,兩者發(fā)生率差異無統(tǒng)計學(xué)意義(χ~2=1.92,P0.05)。側(cè)支循環(huán)通路的主要途徑為I型28例、Ⅱ型2例、Ⅲ型22例,Ⅰ、Ⅲ型側(cè)支循環(huán)通路均顯著多于Ⅱ型,差異有統(tǒng)計學(xué)意義(P0.01)。發(fā)生胃網(wǎng)膜靜脈曲張48例高于其他靜脈曲張的發(fā)生率(P0.001),發(fā)生胃底靜脈曲張11例、食管下段靜脈曲張1例,兩者差異有統(tǒng)計學(xué)意義(χ~2=9.42,P0.01)。NISVO患者中胃底靜脈和結(jié)腸右上靜脈曲張發(fā)生率明顯大于ISVO患者,差異有統(tǒng)計學(xué)意義(χ~2=4.15、7.44,P0.05)。結(jié)論多層螺旋CT顯示胰源性門靜脈高壓側(cè)支循環(huán)具有一定的特征性。
[Abstract]:Objective to analyze the characteristics of collateral circulation of pancreatic portal hypertension by 64 slice spiral CT imaging. Materials and methods the CT portal phase images of 52 patients with pancreatic portal hypertension diagnosed and pathologically confirmed in the affiliated Hospital of Southwest Medical University from August 2013 to May 2015 were retrospectively analyzed and the collateral circulation was observed. Results there were 21 cases of isolated splenic vein occlusion with (ISVO) and 31 cases of non-isolated splenic vein occlusion with (NISVO).
