【摘要】：目的分析胃部異位胰腺多層螺旋CT(MSCT)的影像特征,探討MSCT對胃部異位胰腺的診斷價值。資料與方法回顧性分析15例經(jīng)病理證實的胃部異位胰腺患者的臨床和MSCT資料,從病灶部位、形態(tài)、邊界分析CT表現(xiàn)特征,并測量異位胰腺的大小、異位胰腺及正常胰腺的CT值。結(jié)果 12例異位胰腺位于胃竇,11例位于大彎側(cè);11例向腔內(nèi)生長;10例邊界欠清。病灶的平均長徑(LD)/短徑(SD)值為1.89,異位胰腺大小測量一致性均較高:LD的組內(nèi)相關(guān)系數(shù)為0.99,SD的組內(nèi)相關(guān)系數(shù)為0.98。所有病灶均勻、持續(xù)強化,其中4例病灶邊緣可見臍凹征。異位胰腺與正常胰腺的平掃及門靜脈期CT值差異無統(tǒng)計學意義(t=0.402、-1.05,P0.05),動脈期CT值差異有統(tǒng)計學意義(t=3.13,P0.05)。結(jié)論異位胰腺在MSCT影像上多位于胃竇、大彎側(cè),突向腔內(nèi)生長,呈卵圓形,LD/SD1.4。異位胰腺與正常胰腺密度相似,增強掃描強化程度根據(jù)異位胰腺的病理構(gòu)成而不同。異位胰腺增強掃描可見臍凹征,有助于異位胰腺的診斷與鑒別診斷。
[Abstract]:Objective to analyze the imaging features of multislice spiral CT (MSCT) in ectopic pancreas of stomach and to explore the value of MSCT in the diagnosis of ectopic pancreas. Materials and methods the clinical and MSCT data of 15 patients with gastric ectopic pancreas confirmed by pathology were retrospectively analyzed. The CT features of the ectopic pancreas were analyzed from the location, shape and boundary of the lesion. The size of the ectopic pancreas, the CT value of the ectopic pancreas and the normal pancreas were measured. Results 12 cases of ectopic pancreas were located in antrum of gastric antrum. 11 cases of ectopic pancreas were located in the great curved side. The mean long diameter (LD) / short diameter (SD) of the lesion was 1.89, and the consistency of measurement of ectopic pancreas size was higher than that of the control group. The correlation coefficient was 0.98in the group with 0.99SD. All the lesions were homogeneous and continuously enhanced, of which 4 cases showed umbilical fovea sign at the edge of the lesion. There was no significant difference in CT value between ectopic pancreas and normal pancreas in plain scan and portal vein phase (t = 0.402), but there was significant difference in CT value in arterial phase (t = 3.13 P 0.05). Conclusion the ectopic pancreas is located in antrum, great curved side, and the process grows into the lumen on MSCT images, and it is an oval LD- D / SD 1.4. The density of ectopic pancreas is similar to that of normal pancreas, and the enhancement degree is different according to the pathological composition of ectopic pancreas. The contrast-enhanced scanning of ectopic pancreas shows umbilical fovea sign, which is helpful for the diagnosis and differential diagnosis of ectopic pancreas.
