【摘要】：目的探討冠狀動(dòng)脈追蹤凍結(jié)(SSF)技術(shù)對(duì)高心率(85~135 bpm)受檢者CT冠狀動(dòng)脈成像質(zhì)量的影響。方法搜集冠狀動(dòng)脈CT造影的228例患者,其中188例患者掃描前未服用控制心率藥物;對(duì)照組控制心率患者40例,掃描前常規(guī)服用倍他樂克,減少心率變異性;回顧性心電門控和單扇區(qū)重組掃描技術(shù),原始數(shù)據(jù)使用標(biāo)準(zhǔn)算法(STD)和SSF兩種方法重組,由2名資深醫(yī)師采用雙盲法判讀,采用4分評(píng)分法對(duì)冠狀動(dòng)脈血管每個(gè)階段進(jìn)行評(píng)分,≤3分為可診斷圖像。對(duì)STD和SSF兩種算法數(shù)據(jù)圖像整體質(zhì)量的比較采用卡方檢驗(yàn),對(duì)節(jié)段水平圖像質(zhì)量等級(jí)評(píng)分采用配對(duì)設(shè)計(jì)兩樣本比較的秩和檢驗(yàn)(Wilcoxon兩樣本比較法)分析。結(jié)果 188例患者共評(píng)價(jià)冠狀動(dòng)脈節(jié)段1494段,可顯示節(jié)段1494段,顯示率100%;STD重組算法組冠狀動(dòng)脈可診斷節(jié)段總數(shù)為1415段,不可診斷節(jié)段共79段,總體可診斷率94.7%。SSF重組算法組冠狀動(dòng)脈可診斷節(jié)段為1450段,不可診斷節(jié)段共44段,總體可診斷率97.1%。兩種算法冠脈總體可診斷率比較,其差異有統(tǒng)計(jì)學(xué)意義(χ2=812.02,P0.05)。40例對(duì)照組STD重組算法冠狀動(dòng)脈可診斷節(jié)段總數(shù)為469段,不可診斷節(jié)段共19段,總體可診斷率96.1%。SSF重組算法冠狀動(dòng)脈可診斷節(jié)段總數(shù)為485段,不可診斷節(jié)段共3段,總體可診斷率99.4%。兩種算法冠狀動(dòng)脈總體可診斷率比較,其差異有統(tǒng)計(jì)學(xué)意義(χ2=865.84,P0.05)�；诠�(jié)段水平分析,兩種重組算法圖像質(zhì)量差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論與STD相比,SSF對(duì)中低心率和高心率冠狀動(dòng)脈節(jié)段圖像質(zhì)量均有改善,但對(duì)易受高心率心臟運(yùn)動(dòng)影響的右冠狀動(dòng)脈節(jié)段效果更為顯著,高心率受檢者CT冠脈成像采用SSF技術(shù),可以顯著減輕運(yùn)動(dòng)偽影,提高右冠狀動(dòng)脈圖像質(zhì)量。
[Abstract]:Objective to investigate the effect of frozen coronary artery tracing (SSF) technique on CT coronary angiography in patients with high heart rate (85 ~ 135 bpm). Methods 228 patients with coronary arteriography, 188 patients without heart rate control drugs before scanning, 40 patients with heart rate control group, 40 patients with heart rate control, who were routinely taken betaloc before scanning, to reduce heart rate variability. Retrospective ECG gating and single sector recombination scanning techniques were performed. The original data were reconstructed by two standard algorithms, (STD) and SSF. The two senior physicians interpreted the data by double blind method, and scored each stage of coronary artery by 4 points scoring method. 鈮，
