【摘要】：目的探討MRI動(dòng)態(tài)增強(qiáng)掃描(dynamic contrast-enhancement magnetic resonance imaging,DCE-MRI)與乳腺X線(xiàn)攝影在乳腺病變?cè)\斷中的應(yīng)用效果。方法對(duì)在我院收治的乳腺病變患者應(yīng)用DCE-MRI與乳腺X線(xiàn)攝影進(jìn)行檢查,并就這兩種檢查方法在發(fā)現(xiàn)病變及良惡性腫塊鑒別方面的診斷能力進(jìn)行對(duì)比研究,以期客觀(guān)評(píng)價(jià)兩種檢查方法對(duì)乳腺疾病的診斷效能。結(jié)果以病理確診結(jié)果為金標(biāo)準(zhǔn),計(jì)算乳腺X線(xiàn)攝影診斷良、惡性乳腺病變的敏感性為65.63%,特異性為70.37%,準(zhǔn)確性為68.60%;DCE-MRI診斷良、惡性乳腺病變的敏感性為87.50%,特異性為90.74%,準(zhǔn)確性為89.53%;經(jīng)統(tǒng)計(jì)學(xué)分析發(fā)現(xiàn)DCE-MRI診斷良、惡性乳腺病變的敏感性、特異性及準(zhǔn)確性均明顯高于乳腺X線(xiàn)攝影,且差異具有統(tǒng)計(jì)學(xué)意義。結(jié)論乳腺X線(xiàn)攝影和動(dòng)態(tài)增強(qiáng)MRI對(duì)乳腺病變的鑒別診斷均有一定的實(shí)用價(jià)值,但是乳腺動(dòng)態(tài)增強(qiáng)成像能夠提供更加豐富的形態(tài)學(xué)信息,診斷效能優(yōu)于乳腺X線(xiàn)攝影,兩者的結(jié)合可以發(fā)揮優(yōu)勢(shì)互補(bǔ)的作用。
[Abstract]:Objective to evaluate the effect of MRI dynamic enhanced scanning (DCE-MRI) and mammography in the diagnosis of breast lesions. Methods DCE-MRI and mammography were performed in patients with breast lesions in our hospital, and the diagnostic ability of the two methods in the differential diagnosis of benign and malignant masses was compared. Objective to evaluate the effectiveness of the two methods in the diagnosis of breast diseases. Results according to the gold standard of pathological diagnosis, the sensitivity, specificity and accuracy of DCE-MRI were 65.63, 70.37 and 68.60m respectively. The sensitivity, specificity and accuracy of DCE-MRI in the diagnosis of benign and malignant breast lesions were 87.50%, 90.74 and 89.53, respectively. The sensitivity, specificity and accuracy of DCE-MRI in the diagnosis of benign and malignant breast lesions were significantly higher than those in mammography, and the difference was statistically significant. Conclusion both mammography and dynamic contrast enhanced MRI have some practical value in the differential diagnosis of breast lesions, but dynamic enhanced mammography can provide more abundant morphological information, and the diagnostic efficacy is better than that of mammography. The combination of the two can play a complementary role.
