倍增時間在磨玻璃結(jié)節(jié)隨訪中的應(yīng)用價值

【摘要】:目的探討腫瘤倍增時間(DT)在肺內(nèi)磨玻璃結(jié)節(jié)(GGNs)中的應(yīng)用價值。方法回顧性分析2008年9月至2015年12月在華東醫(yī)院診治的78例肺內(nèi)GGNs的影像學(xué)資料,結(jié)節(jié)均經(jīng)手術(shù)切除,術(shù)后病理證實為微浸潤性腺癌(MIA)和浸潤性腺癌(IAC)。根據(jù)密度差異將初診結(jié)節(jié)分為純磨玻璃結(jié)節(jié)(pGGN)及混合性磨玻璃結(jié)節(jié)(mGGN),采用體積倍增時間(VDT)計算,不同類型結(jié)節(jié)的DT具有統(tǒng)計學(xué)差異。結(jié)果 78例GGNs中共有pGGN40例,mGGN 38例,術(shù)后病理:MIA 38例,IAC 40例。pGGN的平均DT為845天,mGGN的平均DT為538天,使用獨立樣本t檢驗,兩者差異具有統(tǒng)計學(xué)意義(P0.05)。設(shè)定DT≤730天、1095天、1460天,分別作為MIA和IAC的診斷閾值,pGGN診斷為MIA敏感性分別為67%、78%、73%,特異性33%、50%、29%,準(zhǔn)確性50%、60%、56%。設(shè)定DT≤365天、730天、1095天,分別作為MIA和IAC的診斷閾值,mGGN診斷為MIA敏感性分別為25%、27%、24%,特異性50%、88%、89%,準(zhǔn)確性33%、59%、58%。結(jié)論 mGGN較pGGN增長快速,隨訪中應(yīng)縮短隨訪間隔及隨訪時間;初診為pGGN時,以DT≤1095天作為手術(shù)點有較高的價值;初診為mGGN時,以DT≤730天作為手術(shù)點有較高的價值。
[Abstract]:Objective to evaluate the value of tumor doubling time (DT) in (GGNs) of intrapulmonary milled glass nodules. Methods from September 2008 to December 2015, the imaging data of 78 cases of pulmonary GGNs in East China Hospital were retrospectively analyzed. The nodules were surgically resected and pathologically proved to be microinvasive adenocarcinoma (MIA) and invasive adenocarcinoma (IAC). According to the density difference, the newly diagnosed nodules were divided into pure ground glass nodules (pGGN) and mixed ground glass nodules (mGGN),) by volume doubling time (VDT). The DT of different types of nodules was statistically different. Results there were 38 cases of pGGN40 in 78 cases of GGNs.
