【摘要】：目的探討多層螺旋CT(MSCT)掃描及MPR矢狀位重建圖像技術(shù)在胸腰椎壓縮骨折患者影像學(xué)診斷的應(yīng)用價(jià)值。方法選取我院收治的胸腰椎壓縮骨折患者96例為研究對(duì)象,均行X線、MSCT檢查,比較兩種方法診斷胸腰椎壓縮骨折的靈敏度、特異度、準(zhǔn)確度,并分析椎體骨折分布情況及不同類型胸腰椎壓縮骨折的顯示率,觀察胸腰椎壓縮骨折的MSCT影像特征。結(jié)果 96例患者骨折椎體114例,范圍為T12-L5椎體,其中以T12-L1椎體最多(75.44%);MSCT診斷胸腰椎骨折的靈敏度75.00%、準(zhǔn)確度73.96%明顯高于X線檢查60.00%、64.58%(P0.05),特異度比較無顯著差異(P0.05);MPR重建中矢狀位對(duì)胸腰椎壓縮骨折的顯示率90.91%較冠狀位69.42%、橫斷面64.46%高(P0.05);MPR矢狀位重建圖像顯示骨折整體,立體感強(qiáng),能很好地顯示椎管變形、椎管內(nèi)碎骨片。結(jié)論 MSCT掃描及MPR矢狀位重建圖像技術(shù)對(duì)胸腰椎壓縮骨折具有重要的診斷價(jià)值,值得在臨床推廣應(yīng)用。
[Abstract]:Objective to evaluate the value of multislice spiral CT (MSCT) and MPR sagittal reconstruction in the diagnosis of thoracolumbar compression fracture. Methods Ninety-six patients with thoracolumbar compression fractures were selected as subjects. MSCT examination was performed in all patients. The sensitivity, specificity and accuracy of the two methods in the diagnosis of thoracolumbar compression fractures were compared. The distribution of vertebral body fractures and the display rate of different types of thoracolumbar compression fractures were analyzed, and the MSCT imaging features of thoracolumbar compression fractures were observed. Results there were 114 cases of fracture vertebral body in 96 patients with T12-L5 vertebral body. The sensitivity of T12-L1 in diagnosing thoracolumbar fractures was 75.00% (75.44%), and the accuracy of 73.96% was significantly higher than that of X-ray examination of 64.58% (P0.05). There was no significant difference in specificity (P0.05). The display rate of thoracolumbar compression fractures in sagittal position was 90.91% compared with that in coronal 69.42. 64.46% high (P0.05) MPR sagittal reconstruction images showed the fracture as a whole. Strong sense of three-dimensional, can well show deformation of the spinal canal, bone fragments in the spinal canal. Conclusion MSCT scan and MPR sagittal reconstruction are valuable in the diagnosis of thoracolumbar compression fracture.
