第3代雙源CT低管電壓70kV聯(lián)合高級(jí)模擬迭代重建在結(jié)腸CT成像中的應(yīng)用

【摘要】:目的探討第3代雙源CT低管電壓70 kV聯(lián)合高級(jí)模擬迭代重建技術(shù)(ADMIRE)在降低結(jié)腸CT輻射劑量方面的可行性。方法前瞻性連續(xù)入組53例患者行結(jié)腸CT檢查,仰臥位為120 kV標(biāo)準(zhǔn)低劑量掃描,采用濾波反投影法進(jìn)行圖像重建;俯臥位為70 kV掃描,然后采用ADMIRE進(jìn)行圖像重建,掃描范圍一致;比較兩者輻射劑量、圖像質(zhì)量等方面的差異。結(jié)果 70 kV的CT容積劑量指數(shù)、劑量長(zhǎng)度乘積和有效劑量分別為(1.47±0.23)m Gy、(68.85±13.82)m Gy·cm和(1.03±0.21)m Sv,明顯低于120 kV的(2.96±0.61)m Gy(t=25.356,P=0.000)、(136.97±32.77)m Gy·cm(t=22.246,P=0.000)和(2.05±0.49)m Sv(t=22.293,P=0.000)。70 kV聯(lián)合ADMIRE的信噪比為3.60±0.58,明顯高于120 kV的2.34±0.47(t=-18.036,P=0.000);噪聲為(11.81±1.93)HU,明顯低于120 kV的(14.39±2.57)HU(t=7.856,P=0.000)。兩位放射科醫(yī)師主觀評(píng)分總體κ值為0.843(P=0.000)。二維圖像結(jié)果顯示,70 kV聯(lián)合ADMIRE的噪聲明顯低于120 kV(Z=325.000,P=0.000)、圖像總體質(zhì)量評(píng)分明顯高于120 kV(Z=78.000,P=0.001),兩者的銳利度差異無(wú)統(tǒng)計(jì)學(xué)意義(Z=12.000,P=0.180),所有評(píng)分均為4~5分。三維虛擬內(nèi)鏡圖像結(jié)果顯示,70 kV聯(lián)合ADMIRE和120 kV在圖像總體質(zhì)量方面差異無(wú)統(tǒng)計(jì)學(xué)意義(Z=4.000,P=0.059),所有評(píng)分均在4~5分。結(jié)論第3代雙源CT低管電壓70 kV聯(lián)合ADMIRE較120 kV常規(guī)掃描,可在保證結(jié)腸CT圖像質(zhì)量的同時(shí),顯著降低輻射劑量。
