【摘要】：目的探討先天性支氣管閉鎖的多層螺旋CT表現(xiàn)。方法回顧性分析我院收治的15例先天性支氣管閉鎖的病例,男7例,女8例,年齡6~44歲,分析多層螺旋CT上病變的位置、邊界,病灶內(nèi)支氣管及肺血管情況,占位效應(yīng)等特征。結(jié)果先天性支氣管閉鎖發(fā)生于上肺病灶11例,下肺病灶6例。單個(gè)肺段發(fā)生11例,多個(gè)肺段發(fā)生4例,上、下肺同時(shí)發(fā)生2例,上、中肺1例,兩上肺1例。所有病例病灶表現(xiàn)為局限性透亮度增高區(qū),與正常肺組織分界欠清,其內(nèi)肺血管細(xì)小,可見(jiàn)正常肺組織呈尖角狀突入病灶內(nèi)。所有病變內(nèi)均見(jiàn)支氣管擴(kuò)張,合并支氣管黏液栓11例,"指套征"8例。病灶具占位效應(yīng)11例。合并感染3例,先天性肺氣道畸形2例。結(jié)論支氣管閉鎖為支氣管樹(shù)的先天性發(fā)育畸形,可持續(xù)至成人期。當(dāng)肺野內(nèi)局限性肺過(guò)度充氣特別是合并管狀、指套狀密影,以及肺血管細(xì)小時(shí)需考慮到支氣管閉鎖。
[Abstract]:Objective to investigate the multi-slice spiral CT findings of congenital bronchial atresia. Methods Fifteen cases of congenital bronchial atresia in our hospital were retrospectively analyzed, including 7 males and 8 females, aged 644 years. The location, boundary, bronchus and pulmonary vessels of the lesions and the space occupying effect were analyzed on multislice spiral CT. Results congenital bronchial atresia occurred in 11 cases of upper lung lesions and 6 cases of lower lung lesions. There were 11 cases of single lung segment, 4 cases of multiple lung segments, 2 cases of upper and lower lung simultaneously, 1 case of upper, 1 case of middle lung and 1 case of two upper lung. In all cases, the lesion presented as the local hyperluminance area, which was not well separated from the normal lung tissue, and the pulmonary vessels were small, and the normal lung tissue showed a sharp angle into the lesion. Bronchiectasis was found in all lesions, with bronchial mucus embolus in 11 cases and finger cuff sign in 8 cases. There were 11 cases of site occupying effect of diseased cooker. There were 3 cases of complicated infection and 2 cases of congenital pulmonary tract malformation. Conclusion bronchial atresia is a congenital deformity of bronchial tree, which can continue to adult stage. Bronchial atresia should be taken into account when localized lung hyperinflation in the field, especially with tubular, phalanx, and small pulmonary vessels.
