四種CT掃描模式對肺動脈成像圖像質量和輻射劑量的影響

【摘要】:目的比較肺動脈CT血管造影(CTPA)成像中,雙源CT的大螺距Flash模式、雙能模式(DECT)、傳統(tǒng)128層螺旋CT模式(128-CT)及64層CT(64-CT)掃描模式下,圖像成像質量及輻射劑量的差異。資料與方法臨床擬診為肺動脈栓塞的167例患者中,先從Flash模式的患者中隨機選出20例,計算體重指數和平均年齡,以此為標準,選出體重指數和年齡匹配的其他3組不同檢查模式患者,每組20例。由2名醫(yī)師對圖像質量進行5級評分及信噪比(SNR)比較,并比較各組CT容積劑量指數(CTDIvol)、劑量長度乘積(DLP)及有效劑量(ED)。結果 Flash組CTDIvol、DLP和ED分別為(3.72±0.74)m Gy、(137.5±28.7)m Gy·cm、(2.34±0.41)m Sv,DECT組分別為(5.31±1.21)m Gy、(181.6±34.5)m Gy·cm、(3.24±0.57)m Sv,128-CT組分別為(5.66±1.47)m Gy、(198.7±42.1)m Gy·cm、(3.58±0.63)m Sv,64-CT組分別為(6.75±1.68)m Gy、(231.5±54.2)m Gy·cm、(4.21±0.89)m Sv。4種方法對肺動脈不同部位圖像質量主觀評分和SNR比較,差異無統(tǒng)計學意義(P0.05)。結論 4種掃描方式得到的圖像質量無明顯差異,雙源CT的大螺距Flash模式輻射劑量最小,DECT和128-CT模式輻射劑量無明顯差別,64-CT模式輻射劑量最大。
[Abstract]:Objective to compare the imaging quality and radiation dose of dual-source CT with large pitch Flash mode, dual-energy CT mode (DECT), conventional 128-CT mode (128-CT) and 64-slice CT (64-CT) mode in pulmonary artery CT angiography (CTPA) imaging. Materials and methods Twenty out of 167 patients with pulmonary embolism who were clinically diagnosed as pulmonary embolism were randomly selected from the Flash mode to calculate body mass index (BMI) and mean age. The other three groups of patients with body mass index and age matching were selected, 20 patients in each group.
