伏案工作人群職業(yè)性頸肩肌肉疼痛運(yùn)動(dòng)處方的效果

【摘要】:目的:為有效防治伏案工作人群職業(yè)性頸肩肌肉疼痛的發(fā)生與發(fā)展,制定一套該類人群工作中運(yùn)動(dòng)處方并評(píng)價(jià)其效果。方法:篩選符合入選標(biāo)準(zhǔn)的頸肩肌肉疼痛人員44名,隨機(jī)分為實(shí)驗(yàn)組(n=22人)和對(duì)照組(n=22人)。實(shí)驗(yàn)組進(jìn)行運(yùn)動(dòng)處方練習(xí):(1)放松:頭部旋轉(zhuǎn),肩帶部旋轉(zhuǎn);(2)牽伸:前外側(cè)肩頸肌牽伸,后外側(cè)肩頸肌牽伸,肩背部牽伸;(3)力量練習(xí):頸前肌肌力訓(xùn)練,頸后及背部肌力訓(xùn)練,肩背部肌力訓(xùn)練,背部肌力訓(xùn)練。每次練習(xí)20 min,2次/日,5天/周,共6周。對(duì)照組相應(yīng)時(shí)間內(nèi)暫停工作,適當(dāng)休息。運(yùn)動(dòng)干預(yù)前后對(duì)兩組受試者均進(jìn)行VAS疼痛評(píng)分、頸部關(guān)節(jié)活動(dòng)度、上斜方肌最大等長收縮肌力、疲勞檢測,所得數(shù)據(jù)應(yīng)用t檢驗(yàn)進(jìn)行組內(nèi)/間分析。結(jié)果:6周運(yùn)動(dòng)干預(yù)后,實(shí)驗(yàn)組VAS疼痛評(píng)分顯著低于對(duì)照組(P0.001);頸椎左右側(cè)屈、左右旋轉(zhuǎn)活動(dòng)度均顯著大于對(duì)照組(P0.05),其他各指標(biāo)兩組無顯著性差異。實(shí)驗(yàn)組干預(yù)前后組內(nèi)VAS評(píng)分顯著性降低(P0.001);左右側(cè)屈關(guān)節(jié)活動(dòng)度顯著增大(P0.01);上斜方肌最大收縮肌力顯著增大(P0.01)。對(duì)照組運(yùn)動(dòng)干預(yù)前后各指標(biāo)均無顯著變化(P0.05)。結(jié)論:以頸肩背部肌肉放松、牽伸、等長收縮練習(xí)為主的工作中運(yùn)動(dòng)處方簡便、有效、可行,通過增強(qiáng)頸肩背部關(guān)節(jié)肌肉功能,能有效改善伏案工作人群頸肩肌肉疼痛癥狀及頸椎活動(dòng)情況。
[Abstract]:Objective: to establish a set of exercise prescriptions and evaluate the effect of occupational neck and shoulder muscle pain in the working population in order to prevent the occurrence and development of occupational neck and shoulder muscle pain. Methods: 44 patients with neck and shoulder muscle pain were randomly divided into experimental group (n = 22) and control group (n = 22). Exercise prescription exercise 1) relaxation: head rotation, shoulder band rotation (2) stretching: anterolateral shoulder neck muscle, posterolateral shoulder neck muscle drafting, shoulder and back muscle stretching 3) strength exercises: anterior cervical muscle strength training, neck and back muscle strength training, Shoulder back strength training, back strength training. Practice for 20 mins / day for 5 days / week for 6 weeks. In the control group, the work was suspended and the rest was appropriate. Before and after exercise intervention, VAS pain score, neck joint activity, maximum isometric contraction of superior trapezius muscle and fatigue were measured. The data were analyzed by t test. Results after 6 weeks of exercise intervention, the VAS pain score in the experimental group was significantly lower than that in the control group (P 0.001), and the flexion of the cervical spine on the left and right side was significantly higher than that in the control group (P 0.05). There was no significant difference in other indexes between the two groups. Before and after intervention, the VAS score of the experimental group decreased significantly (P 0.001), the flexion joint activity of the left and right sides increased significantly (P 0.01), and the maximal contractile strength of the superior trapezius muscle increased significantly (P 0.01). In the control group, there was no significant change in every index before and after exercise intervention (P 0.05). Conclusion: the exercise prescription is simple, effective and feasible in the exercise of relaxation, stretching and isometric contraction of the neck, shoulder and back muscles, by enhancing the muscle function of the neck, shoulder and back joint. It can effectively improve the symptoms of neck and shoulder muscle pain and cervical vertebrae movement in the working population.
