常規(guī)輻射劑量評估參數(shù)與體型特異性劑量評估在估算兒童心臟CT掃描輻射劑量的差異

【摘要】:目的比較常規(guī)輻射劑量評估參數(shù)與體型特異性劑量評估(SSDE)在估算先天性心臟病(CHD)患兒CT掃描時所受輻射劑量的差異。方法將161例CHD患兒按年齡分為A組(4個月)、B組(4個月~1歲)、C組(1~6歲)、D組(6歲),采用Philips 128螺旋CT掃描儀對CHD患兒進(jìn)行心臟CT增強(qiáng)掃描,在氣管隆突層面測量每例患兒的左右徑(LAT)、前后徑(AP),測算患兒有效直徑(ED),同時記錄每例患兒的容積CT劑量指數(shù)(CTDIvol)值、劑量長度乘積(DLP)值、有效輻射劑量(ED*)值,計算SSDE。分析CTDIvol、DLP、ED*及SSDE之間的差異性。結(jié)果 A組CTDIvol和SSDE分別為(2.705±0.110)m Gy和(6.688±0.352)m Gy(t=-65.780,P0.001),B組CTDIvol和SSDE分別為(2.699±0.166)m Gy和(6.394±0.429)m Gy(t=-66.191,P0.001),C組CTDIvol和SSDE分別為(2.516±0.232)m Gy和(5.727±0.640)m Gy(t=-32.653,P0.001),D組CTDIvol和SSDE分別為(2.580±0.147)m Gy和(5.098±0.289)m Gy(t=-15.540,P0.001),SSDE值均大于CTDIvol值,且隨著年齡增長差值越大,SSDE之間差值較CTDIvol之間差值更大。另采用Pearson相關(guān)分析對體重及CTDIvol、DLP、ED*、SSDE進(jìn)行相關(guān)性分析,結(jié)果顯示體重與CTDIvol之間無相關(guān)性(r=-0.138,P0.05),而與DLP、ED*、SSDE存在相關(guān)性(r分別為0.232、0.232、-0.422,P0.01)。結(jié)論 CTDIvol明顯低估了患兒所受的輻射劑量,SSDE能夠更準(zhǔn)確地反映不同體型患兒進(jìn)行心臟CT掃描時實際接受的輻射劑量。
[Abstract]:Objective to compare the difference of radiation dose between conventional radiation dose evaluation parameters and somatotype specific dose evaluation (SSDE) in estimating CT scan dose in children with congenital heart disease (CHD). Methods 161 children with CHD were divided into group A (4 months old) and group B (4 months old) and group D (6 years old) were divided into group A (4 months old) and group D (6 years old).
