二代雙源CT大螺距肺動(dòng)脈成像低劑量與低對(duì)比劑用量的應(yīng)用研究

【摘要】:目的:評(píng)估二代雙源CT大螺距掃描方式在低劑量和低對(duì)比劑個(gè)體化肺動(dòng)脈成像中的可行性。方法:按掃描方式和對(duì)比劑劑量不同將90例患者分成A、B、C三組。A組30例,采用傳統(tǒng)肺動(dòng)脈成像檢查方案,對(duì)比劑用量70mL,管電壓100kV,螺距1.2;B組和C組采用低對(duì)比劑劑量30mL,大螺距2.1,根據(jù)預(yù)檢測(cè)層面左心室衰減噪聲值≥15HU和15HU分為B組和C組,管電壓分別為100kV、80kV。分析比較三組的圖像質(zhì)量及輻射劑量。結(jié)果:三組患者年齡、性別均無(wú)明顯差異;C組體重低于A、B兩組(P0.01)。三組掃描時(shí)間、掃描長(zhǎng)度、容積CT劑量指數(shù)(CTDIvol)、有效劑量(ED)差異均有統(tǒng)計(jì)學(xué)意義(P0.01)。三組圖像治療均滿足診斷要求。A組雙側(cè)肺動(dòng)脈強(qiáng)化CT值最高,B組最低。A組上腔靜脈內(nèi)對(duì)比劑線束硬化偽影比例高于B組和C組(P0.05)。兩位醫(yī)師對(duì)所有病例圖像的評(píng)分有良好的一致性(Kappa=0.719,U=8.02,P0.05)。結(jié)論:采用二代雙源CT大螺距肺動(dòng)脈掃描并根據(jù)左心室平掃衰減噪聲值來(lái)調(diào)整管電壓的個(gè)體化肺動(dòng)脈成像能夠明顯降低患者的輻射劑量以及對(duì)比劑用量,可以作為臨床肺動(dòng)脈栓塞患者的首選篩查和隨訪檢查方法。
[Abstract]:Objective: to evaluate the feasibility of two generation double source CT large pitch scan in low dose and low contrast medium individualized pulmonary artery imaging.
