低劑量CT灌注成像在腦梗死患者恢復(fù)期的應(yīng)用

【摘要】:目的:研究低劑量CT灌注成像顯示腦梗死患者恢復(fù)期階段梗死灶腦細(xì)胞活性的診斷價(jià)值。方法:選取50例臨床診斷為大面積腦梗死的患者,在腦梗死恢復(fù)期隨機(jī)分成兩組,在做常規(guī)平掃基礎(chǔ)上分別行常規(guī)劑量(120 k V、200 m As)和低劑量(80 k V、200 m As)螺旋CT灌注掃描成像,由2名影像科資深專家評(píng)價(jià)圖像并做出正確的診斷。結(jié)果:CT灌注成像除可以發(fā)現(xiàn)常規(guī)CT平掃和增強(qiáng)平掃能發(fā)現(xiàn)的陳舊性梗死灶外,還可以發(fā)現(xiàn)明顯的梗死區(qū)周圍局部血流增加現(xiàn)象。低劑量組所接受的輻射劑量遠(yuǎn)低于常規(guī)劑量,低劑量與常規(guī)劑量CT灌注成像下梗死中心區(qū)、缺血區(qū)各灌注參數(shù)值比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:低劑量CT灌注成像在降低輻射劑量的同時(shí)可較好地評(píng)價(jià)梗死區(qū)局部血流灌注改善程度,對(duì)臨床判斷預(yù)后及治療有著重要的意義。
[Abstract]:Objective: to study the diagnostic value of low dose CT perfusion imaging in the diagnosis of cerebral infarction in convalescent stage. Methods: fifty patients with large area cerebral infarction were selected and randomly divided into two groups during the convalescence of cerebral infarction. On the basis of routine plain scan, the conventional CT perfusion imaging was performed at the dose of 120kV / 200mAsand low dose of 80kV / 200mAsrespectively. The images were evaluated by two senior experts in the department of imaging and correct diagnosis was made. Results in addition to conventional CT plain scan and enhanced CT perfusion imaging, the old infarct foci could be found as well as the increased regional blood flow around the infarct area. The radiation dose in the low dose group was much lower than that in the conventional dose group. There was no significant difference in the perfusion parameters of the infarct center and ischemic area between the low dose group and the conventional dose CT perfusion imaging group (P 0.05). Conclusion: low dose CT perfusion imaging can reduce the radiation dose and evaluate the improvement of regional blood flow perfusion in the infarcted area, which is of great significance for clinical prognosis and treatment.
