即刻種植和延期種植后種植體周圍骨質(zhì)變的錐形束CT的對(duì)比研究

【摘要】:目的:利用錐形束CT(CBCT)影像分析來比較即刻種植和延期種植后種植體周圍的骨質(zhì)變化。 材料和方法:實(shí)驗(yàn)動(dòng)物選用24月齡雄性雜種犬六條,體重10-15公斤。麻醉下拔除實(shí)驗(yàn)犬雙側(cè)上頜第三切牙(I3),第3和第4下頜前磨牙(P3和P4),每個(gè)P3近中拔牙窩和P4遠(yuǎn)中拔牙窩被選定為實(shí)驗(yàn)組,而其余拔牙窩作為不進(jìn)行干預(yù)自然愈合的對(duì)照組。種植體植入?yún)^(qū)域會(huì)隨機(jī)接受如下治療方案中的一種:(1)即刻種植(IIP)和即刻負(fù)重(IL)組;(2)延期種植(DIP)和延期負(fù)重(DL組);(3)延期種植(DIP)和即刻負(fù)重(IL)組;(4)對(duì)照組(拔牙窩自然愈合)。最終處死實(shí)驗(yàn)犬,取材、固定,在選定ROI區(qū)域范圍內(nèi)利用CBCT進(jìn)行三維重建得到骨小梁空間結(jié)構(gòu)的計(jì)算機(jī)三維圖形,對(duì)樣本進(jìn)行三維評(píng)測(cè),指標(biāo)包括:骨體積(BV),組織體積(TV),骨體積分?jǐn)?shù)(BV/TV),骨表面積(BS),骨表面積體積比(BS/BV),骨小梁厚度(Tb.Th),骨小梁間隙(Tb.Sp),骨小梁數(shù)目(Tb.N),,結(jié)構(gòu)模型指數(shù)(SMI),骨小梁模式因子(TbPf),分形維數(shù)(FD)和Conn.D.連接密度。 結(jié)果:分析結(jié)果表明IIP+IL, DIP+IL和DIP+DL實(shí)驗(yàn)組樣本中的骨表面積體積比(BS/BV)、分形維數(shù)(FD)、骨小梁間隙(Tb.Sp)和接觸密度(Conn.Dn)與對(duì)照組有明顯差異(P0.05)。其中所有實(shí)驗(yàn)組骨表面積體積比(BS/BV)和接觸密度(Conn.Dn)比對(duì)照組均數(shù)數(shù)值差異明顯高,分形維數(shù)(FD)和骨小梁間隙(Tb.Sp)比對(duì)照組均數(shù)數(shù)值差異明顯低。而三組實(shí)驗(yàn)組之間的比較:IIP+IL vs.DIP+IL; IIP+IL vs. DIP+DL; DIP+IL vs. DIP+DL.之間無明顯差異(P0.05)。 結(jié)論: 1.該動(dòng)物實(shí)驗(yàn)結(jié)果表明與非種植區(qū)相比,種植體周圍骨的質(zhì)量顯著提高。 2.即刻種植即刻負(fù)重(IIP+IL)、延期種植延期負(fù)重(DIP+DL)、延期種植即刻負(fù)重(DIP+IL)三種治療方案中種植體周圍骨無差異。
[Abstract]:Aim: to compare the changes of bone around implants after immediate implantation and delayed implantation by CT-CBCT image analysis. Materials and methods: six 24-month-old male hybrid dogs weighing 10-15 kg were used. The third and fourth mandibular premolars were extracted under anesthesia. The third and fourth mandibular premolars were selected as the experimental group. Each P3 proximal extraction fossa and P4 distal extraction fossa were selected as the experimental group, while the other extraction fossa were used as the control group without intervention in natural healing. Implants were randomly assigned to one of the following treatments: 1) immediate implant (IIP1) and 2) immediate weight bearing (ILL) group) and 2) delayed implant group (DIP3) and (2) DIP3) group) and 4) control group (natural healing of tooth extraction nest). Finally, the experimental dogs were killed, the specimens were taken and fixed, and the three-dimensional reconstruction of the bone trabecular space structure was made by using CBCT within the selected ROI area, and the three-dimensional evaluation of the samples was carried out. The indexes include: bone volume, tissue volume, bone volume fraction, bone surface area, bone surface area ratio, bone trabecular thickness, trabecular space, bone trabecular number, Tb.N, structural model index, trabecular pattern factor TbPfU, fractal dimension FDD and Conn.D. Connection density. Results: the results showed that the volume ratio of bone surface area of IIP IL, DIP IL and DIP DL group was significantly different from that of control group (P 0.05). The fractal dimension of bone surface area and bone trabecular space (Tb.Spn) and contact density (Conn.Dn) were significantly different from those of control group (P < 0.05). The bone surface area ratio (BS / BV) and contact density (Conn.Dn) of all experimental groups were significantly higher than those of the control group, and the fractal dimension (FDD) and trabecular space (TB. Sp) were significantly lower than those of the control group. The comparison among the three experimental groups was made between the three groups: IIP IL vs.DIP IL; IIP IL vs. DIP DL; DIP IL vs. DIP DL. There was no significant difference between the two groups (P 0.05). Conclusion: 1. The results of the animal experiment showed that the quality of bone around the implant was significantly higher than that in the non-implant area. 2. There was no difference in the periimplant bone among the three treatments: immediate implantation of IIP ILA, delayed implantation of DIP DLX, and delayed implantation of immediate load bearing DIP.
