【摘要】：近年中藥電子處方的普及為中醫(yī)臨床工作帶來極大的便利,由其帶來的審方模式改變、軟件設(shè)計缺陷等問題,導(dǎo)致中藥電子處方評價出現(xiàn)諸多問題,因此如何構(gòu)建中藥電子處方評價體系日益受到關(guān)注。 論文包括文獻綜述和臨床藥學(xué)研究兩部分。 綜述部分系統(tǒng)總結(jié)了傳統(tǒng)中藥處方評價和中藥電子處方評價的內(nèi)容與方法,闡述了中藥電子處方的利弊及發(fā)展方向。中藥電子處方取藥流程發(fā)生改變先交費后審方,因此存在修改處方繁瑣、錯誤輸入增多、軟件設(shè)計不夠“中醫(yī)化”等弊病,其未來發(fā)展動向有：開發(fā)合理用藥自動提示系統(tǒng)、開發(fā)中藥電子處方自動監(jiān)測系統(tǒng)以及中藥電子處方標(biāo)準化建設(shè)等。 臨床藥學(xué)研究包括兩章內(nèi)容。第一章“海軍總醫(yī)院中藥處方管理現(xiàn)狀及分析”,該章節(jié)首先對海軍總醫(yī)院信息系統(tǒng)的利弊進行了分析。其次隨機抽取海軍總醫(yī)院2010年度門診中藥電子處方3600張,以《中華人民共和國藥典》《處方管理辦法》為標(biāo)準,從藥物劑量、特殊用法、配伍禁忌、藥味數(shù)量及用藥方法等5方面對其進行統(tǒng)計分析。結(jié)果：中藥處方用藥劑量不合格率為90.61%,特殊用法不合格率為29.33%,配伍禁忌不合格率為0.75%,用藥方法不合格率為6.17%。處方中10味以下處方占總處方比例為19.33%,10-17味藥處方占總處方比例為57.08%,18-21味處方占總處方比例為14.67%,22味以上處方占總處方比例為8.92%。超劑量用藥前20味藥物有清半夏、當(dāng)歸、柴胡、醋五味子、麥冬、首烏藤、丹參、酒萸肉、白術(shù)、郁金、蜜桑白皮、法半夏、黃連等。特殊用法中未腳注的前10味藥物分別是薄荷、蜜枇杷葉、石斛、牡蠣、龍骨、砂仁、降香、檀香、石膏、鉤藤。配伍禁忌中違反“十八反”、“十九畏”的處方有27張。結(jié)果顯示,海軍總醫(yī)院存在中藥處方不規(guī)范現(xiàn)象,而通過構(gòu)建中藥處方評價體系,可以推動海軍總醫(yī)院中藥處方向規(guī)范化、標(biāo)準化方向發(fā)展。 第二章“海軍總醫(yī)院中藥處方評價體系構(gòu)建”。目前絕大部分中藥電子處方評價方法仍為手工評價,有些醫(yī)院甚至尚未建立處方點評制度,且絕大多數(shù)的處方評價是回顧性檢驗,在處方的調(diào)劑與發(fā)放流程,無法修正不合理用藥,因此亟需構(gòu)建中藥電子處方分析和評價體系。該章節(jié)以《中華人民共和國藥典》(2010年版一部)、《北京中藥飲片炮制規(guī)范》(2008年版)、《《中藥大辭典》為標(biāo)準,結(jié)合海軍總醫(yī)院中藥電子處方分析結(jié)果,以Microsoft Viaual Studio2010為開發(fā)環(huán)境,以Access為后臺數(shù)據(jù)庫,以C#語言為開發(fā)語言。校正中藥處方用名,構(gòu)建中藥基本標(biāo)準信息體系,其中包括中藥標(biāo)準用量信息、配伍及妊娠禁忌標(biāo)準信息、特殊用法標(biāo)準信息,并在現(xiàn)有的中藥飲片代碼基礎(chǔ)上增加中藥功效碼,構(gòu)建新的中藥飲片代碼。在以上工作基礎(chǔ)上構(gòu)建醫(yī)生工作站中的處方提醒信息系統(tǒng)、評價體系用法幫助系統(tǒng)和藥房工作站中的電子處方評價系統(tǒng)。初步臨床應(yīng)用顯示本評價體系可提高醫(yī)生開具處方的一次性合格率、提高藥房審方的工作效率和準確度,處方評分還可為醫(yī)生處方質(zhì)量評價提供客觀參考。 本課題首次構(gòu)建了中藥電子處方評價體系,部分解決了海軍總醫(yī)院中醫(yī)電子處方評價中存在的問題,提高了工作效率、提升了用藥安全和中醫(yī)信息化水平。該研究結(jié)果可為全國范圍內(nèi)的中藥電子處方評價體系構(gòu)建、中藥電子處方標(biāo)準化建設(shè)提供有益的參考。
[Abstract]:The popularization of electronic prescription of Chinese medicine in recent years has brought great convenience to the clinical work of traditional Chinese medicine . It has caused many problems in the evaluation of electronic prescription of traditional Chinese medicine due to the change of the mode of trial prescription and the defect of software design , so how to build the electronic prescription evaluation system of traditional Chinese medicine is becoming more and more attention .

The article includes literature review and clinical pharmaceutical research .

This paper summarizes the contents and methods of traditional Chinese medicine prescription evaluation and electronic prescription evaluation of traditional Chinese medicine , and expounds the advantages and disadvantages of traditional Chinese medicine electronic prescription and the developing direction of traditional Chinese medicine electronic prescription .

This paper analyzes the advantages and disadvantages of the traditional Chinese medicine prescription in the general hospital of the Navy . The results show that the prescription of the traditional Chinese medicine prescription is 90 . 61 % , the proportion of the special usage is 29.33 % , the prescription proportion of the prescription is 14.67 % , the prescription of the medicine is 6.17 % .

In chapter 2 , the traditional Chinese medicine prescription evaluation system is established . Most of the traditional Chinese medicine electronic prescription evaluation methods still have manual evaluation , some hospitals have not even established the prescription evaluation system , and most of the prescription evaluation is the development environment of the traditional Chinese medicine prescription .

The paper constructs the electronic prescription evaluation system for Chinese medicine for the first time , partially solves the problems existing in the traditional Chinese medicine electronic prescription evaluation of the navy general hospital , improves the working efficiency , improves the medicine safety and the traditional Chinese medicine informatization level , and can provide beneficial reference for the construction of the traditional Chinese medicine electronic prescription evaluation system in the whole country , and the standardized construction of the electronic prescription of the traditional Chinese medicine .
