【摘要】：目的通過探討雙低劑量在頭頸部無迭代技術多層螺旋CT血管成像(CTA)應用中的可行性,實現(xiàn)在降低輻射劑量的基礎上減少對比劑使用量,以降低受檢者輻射危害和對比劑腎病發(fā)病率。資料與方法選取51例行雙低劑量頭頸CTA檢查的患者作為雙低組[管電壓100 kV,管電流300 m A,對比劑0.7 ml/kg碘佛醇(320mg I/ml)],51例行常規(guī)劑量頭頸CTA檢查的患者作為常規(guī)組[管電壓120 kV,管電流350 m A,對比劑1.0 ml/kg碘佛醇(320 mg I/ml)]。測量兩組患者血管內(nèi)CT值和背景噪聲,比較兩組患者信噪比(SNR)、對比噪聲比(CNR)、圖像質(zhì)量主觀評分、輻射劑量及碘負荷量。結(jié)果雙低組與常規(guī)組容積CT劑量指數(shù)分別為18.00 m Gy、33.86 m Gy,有效劑量分別為3.85 m Sv/(m Gy·cm)、7.25 m Sv/(m Gy·cm),碘負荷量分別為224 mg I/kg、320 mg I/kg。雙低組主動脈弓SNR及CNR高于常規(guī)組,但差異無統(tǒng)計學意義(t=-1.572、-1.783,P0.05);雙側(cè)頸總動脈、頸內(nèi)動脈、大腦中動脈SNR及CNR均低于常規(guī)組,但差異無統(tǒng)計學意義(t=0.341~1.739,P0.05)。雙低組及常規(guī)組圖像質(zhì)量主觀評分分別為(3.69±0.47)分、(3.70±0.46)分,差異無統(tǒng)計學意義(Z=-0.213,P0.05)。結(jié)論采用雙低劑量掃描技術在無迭代技術螺旋CT行頭頸部CTA檢查可以獲得質(zhì)量滿意的圖像。
[Abstract]:Objective to explore the feasibility of the application of double low dose in multislice spiral CT angiography (CTAA) without iterative technique in head and neck, so as to reduce the dosage of contrast agent on the basis of reducing radiation dose. In order to reduce the radiation hazards and the incidence of contrast nephropathy. Materials and methods 51 patients with double low dose head and neck CTA were selected as the double low group [tube voltage 100kV, tube current 300mA, contrast agent 0.7 ml/kg iodovolol 320mg I / ml] and 51 patients with routine dose head and neck CTA examination as routine group [tube] The voltage is 120kV, the current of the tube is 350mA, the contrast agent is 1.0 ml/kg iodovolol 320mg / ml]. Intravascular CT value and background noise were measured in two groups. The SNR of two groups were compared. The ratio of noise to noise, the subjective score of image quality, the dose of radiation and the amount of iodine load were compared. Results the volume CT dose index was 18.00mGy 33.86 mGy, the effective dose was 3.85m Sv/(m Gy CMY 7.25m Sv/(m Gy CMY, and the iodine load was 224mg / kg ~ 320 mg / kg ~ (-1) 路kg ~ (-1) ~ (-1) ~ (-1) ~ (-1) ~ (-1) ~ (-1) ~ (-1) ~ (-1) ~ (-1) ~ (-1) ~ (-1), respectively. The SNR and CNR of aortic arch in the double low group were higher than those in the routine group, but there was no significant difference between the two groups, but the SNR and CNR of the bilateral common carotid artery, internal carotid artery, middle cerebral artery and middle cerebral artery were lower than those of the routine group, but the difference was not statistically significant. The subjective score of image quality in double low group and routine group was 3.69 鹵0.47) and 3.70 鹵0.46, respectively. The difference was not statistically significant (P 0.05). Conclusion double low dose scanning technique in spiral CT without iterative technique can obtain satisfactory quality of head and neck CTA.
