【摘要】:目的:探討生物等效均勻劑量優(yōu)化算法在調(diào)強(qiáng)放療中對正常組織的保護(hù)作用,為臨床調(diào)強(qiáng)計劃設(shè)計優(yōu)化提供更好的經(jīng)驗。方法:隨機(jī)抽取鼻咽癌、食管癌和直腸癌各20例患者作為研究對象,分別對患者設(shè)計兩組治療計劃進(jìn)行調(diào)強(qiáng)優(yōu)化。1組對危及器官增設(shè)等效均勻劑量(Equivalent Uniform Dose,EUD)生物優(yōu)化(Plan-EUD)條件限制,另一組進(jìn)行常規(guī)物理參數(shù)優(yōu)化(Plan-noEUD)。采用統(tǒng)計學(xué)方法對兩組計劃正常組織劑量限制作用進(jìn)行比較。結(jié)果:Plan-EUD組優(yōu)化后腮腺平均劑量為2737 cGy,平均V_(30)為39.5%,平均并發(fā)癥概率(NTCP)為0.29,Plan-noEUD組優(yōu)化后相應(yīng)值分別為3087 cGy、46.2%、0.38。EUD優(yōu)化平均劑量下降350 cGy,平均V_(30)下降6.7%,NTCP下降0.09,EUD優(yōu)化明顯利于腮腺的保護(hù)。結(jié)論:生物等效均勻劑量優(yōu)化算法在大幅提升病灶靶區(qū)邊緣劑量,提高靶區(qū)的適形度和劑量均勻性的同時,可以顯著降低正常組織受量,使正常組織得到更好保護(hù)。
[Abstract]:Objective: to investigate the protective effect of bioequivalent uniform dose optimization algorithm on normal tissue in intensity modulated radiotherapy (IMRT), and to provide better experience for the optimization of clinical IMRT design. Methods: 20 patients with nasopharyngeal carcinoma, 20 patients with esophageal carcinoma and 20 patients with rectal cancer were randomly selected as study subjects. The patients were selected to design two treatment plans to optimize the condition of Plan-EUD. The other group was optimized by conventional physical parameters. A statistical method was used to compare the effects of normal tissue dose limitation between the two groups. Results the average dose of parotid gland was 2737 cGy, and the average number of venom 30 was 39.5 in the group of: Plan-EUD. The average probability of complication was 0.29. The corresponding values of the optimized average dose of 0.38.EUD were 3087 cGy ~ 46.2g / 0.38.EUD decreased by 350cGy, and the decrease of 6.7NTCP by 0.09cGy was beneficial to the protection of parotid gland. Conclusion: the bioequivalent uniform dose optimization algorithm can significantly increase the edge dose and the conformability and dose uniformity of the target area, at the same time, it can significantly reduce the normal tissue dose, and make the normal tissue better protection.
