【摘要】：目的探討運(yùn)用CT引導(dǎo)下射頻熱凝治療原發(fā)性三叉神經(jīng)痛的臨床效果。方法選取原發(fā)性三叉神經(jīng)痛患者165例作為研究對(duì)象,所有患者采用隨機(jī)抽簽法分成射頻熱凝組和藥物毀損組,射頻熱凝組患者83例,藥物毀損組患者82例。射頻熱凝組采用CT引導(dǎo)下射頻熱凝治療三叉神經(jīng)痛,藥物毀損組采用CT引導(dǎo)下經(jīng)皮穿刺卵圓孔表阿霉素化學(xué)毀損治療三叉神經(jīng)痛,于治療后3個(gè)月、6個(gè)月、1年、3年進(jìn)行隨訪,觀察兩組患者的VAS評(píng)分變化、治療總有效率及并發(fā)癥發(fā)生率指標(biāo)。結(jié)果射頻熱凝組優(yōu)良率明顯優(yōu)于藥物毀損組,兩組差異有統(tǒng)計(jì)學(xué)意義(P0.05);兩組并發(fā)癥發(fā)生率比較,射頻熱凝組明顯更低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);兩組隨訪期VAS疼痛評(píng)分比較,觀察組更低,且遠(yuǎn)期疼痛情況穩(wěn)定,總體治療效果突出。結(jié)論采用CT引導(dǎo)下射頻熱凝治療原發(fā)性三叉神經(jīng)痛的臨床效果顯著,值得推廣應(yīng)用。
[Abstract]:Objective to investigate the clinical effect of CT-guided radiofrequency thermocoagulation in the treatment of primary trigeminal neuralgia. Methods 165 patients with primary trigeminal neuralgia were randomly divided into radiofrequency thermocoagulation group (RFA) group (n = 83) and drug lesion group (n = 82). Radiofrequency thermocoagulation group was used to treat trigeminal neuralgia under CT guidance, and drug lesion group was treated by percutaneous puncture of adriamycin on oval foramen under CT guidance. The patients were followed up for 3 months, 6 months, 1 year and 3 years after treatment. The changes of VAS score, the total effective rate and the incidence of complications in the two groups were observed. Results the excellent and good rate of radiofrequency thermocoagulation group was significantly better than that of the drug lesion group, and the difference between the two groups was statistically significant (P 0.05), the incidence of complications in the radiofrequency thermocoagulation group was significantly lower than that in the radiofrequency thermocoagulation group (P 0.05), and the VAS pain score in the two groups during the follow-up period was higher than that in the control group. The observation group is lower, and the long-term pain is stable, the overall therapeutic effect is outstanding. Conclusion CT guided radiofrequency thermocoagulation is effective in the treatment of primary trigeminal neuralgia.
