【摘要】：目的:探討十二指腸異位胰腺的臨床特點(diǎn)和CT表現(xiàn)。方法:回顧性分析影像學(xué)誤診但經(jīng)病理證實(shí)的10例十二指腸異位胰腺患者的臨床及影像資料,分析患者的主要臨床表現(xiàn)、病變部位、病變形態(tài)特征、長徑(LD)與短徑(SD)比值范圍,以及病灶與正常胰腺組織CT值的差異性。結(jié)果:主要臨床表現(xiàn):反復(fù)腹痛伴腹脹5例,黑便3例,黃疸2例;影像學(xué)表現(xiàn):2例病灶未見顯影,分別位于十二指腸乳頭部和Vater壺腹;8例可見病灶均表現(xiàn)為位于Vater壺腹以上的結(jié)節(jié)性病灶,其中6例基底部呈山田Ⅱ型,7例腔內(nèi)生長,5例密度均勻且邊界清晰;8例可見病灶LD/SD值:1.14~1.92,平均1.52±0.24,5例LD/SD1.4;病灶實(shí)質(zhì)區(qū)平掃及靜脈期CT值稍低于同層正常胰腺組織(t=-3.41,-1.00,P0.05),差值均數(shù)僅分別為(6.38±2.45)HU、(5.25±3.33)HU,肉眼觀難以分辨;誤診情況:4例誤診為平滑肌瘤或炎性息肉,4例誤診為間質(zhì)瘤;2例誤診為壺腹周圍癌。結(jié)論:十二指腸異位胰腺影像學(xué)多表現(xiàn)為寬基底、腔內(nèi)生長、邊界清晰、密度均勻、強(qiáng)化方式及程度與正常胰腺基本一致的結(jié)節(jié),但當(dāng)病灶表現(xiàn)不典型(如邊界不清、密度不均、未見顯影)時(shí)易造成誤診。臨床工作中加強(qiáng)對(duì)本病的認(rèn)識(shí),充分結(jié)合臨床表現(xiàn),有助于做出提示性診斷。
[Abstract]:Objective: to investigate the clinical features and CT findings of duodenal ectopic pancreas.Methods: the clinical and imaging data of 10 patients with duodenal ectopic pancreas who were misdiagnosed by imaging and proved by pathology were retrospectively analyzed. The main clinical manifestations, pathological location, pathological morphology and the ratio of LDD to SD1 were analyzed.And the difference of CT value between the lesion and normal pancreatic tissue.Results: the main clinical manifestations were repeated abdominal pain with abdominal distension in 5 cases, black stool in 3 cases, jaundice in 2 cases, and no imaging findings in 2 cases.The lesions in the duodenal head of the mammary gland and in the ampulla of Vater were all nodular lesions located above the ampulla of Vater.Of them, 6 cases showed Yamada 鈪，
