顯微CT在骨腱結(jié)合部損傷中的應用與低強度循環(huán)載荷對損傷的影響

【摘要】:髕骨髕腱骨腱結(jié)合部損傷是常見的運動損傷,是當今體育科學界屆的熱點問題。在整個對髕骨髕腱骨腱結(jié)合部損傷機理及治療手段的研究中,動物實驗是其中重要的環(huán)節(jié)。本研究在Nakama循環(huán)載荷動物模型以及前階段實驗的基礎上,研究閾下強度運動對骨腱結(jié)合部損傷的影響,并通過實驗探討Micro-CT技術(shù)計算纖維軟骨帶厚度的可行性,同時使用Micro-CT技術(shù)計算實驗組和對照組的骨量和骨小梁變化。 16只成年雌性新西蘭大白兔,隨機篩選分組分為4周循環(huán)載荷組(n=8),8周循環(huán)載荷組(n=8),左后肢進行實驗,右后肢為空白對照。實驗過程中,前4周兩組實驗動物統(tǒng)一接受強度為7.17N/S的肌肉收縮刺激,一周3次,一次2小時,共24小時。實驗4周后,4周循環(huán)載荷組停止實驗,8周循環(huán)載荷組將強度改為4.2N/S,實驗時間不變。8周后統(tǒng)一進行取材,將取材后組織首先進行Micro-CT進行掃描圖像重建,隨后進行固定、脫鈣、脫水、石蠟包埋、切片和HE染色制成組織切片,使用圖像采集系統(tǒng)進行組織學分析。 結(jié)果:對比對照組,8周循環(huán)載荷組和4周循環(huán)載荷組均在髕腱出現(xiàn)了玻璃樣變,纖維紊亂;潮線出現(xiàn)“漲潮”、模糊乃至消失,纖維軟骨帶增厚,顯示損傷模型建立成功。HE染色測量纖維軟骨帶厚度8周循環(huán)載荷組和4周循環(huán)載荷組沒有顯著差異,Micro-CT計算纖維軟骨帶厚度結(jié)果與HE染色測量結(jié)果未見相關(guān)性,通過Micro-CT計算骨量和骨小梁結(jié)果顯示實驗組和對照組沒有顯著差異。結(jié)論:①對髕骨骨腱結(jié)合部損傷實施閾下強度載荷訓練和休息的干預手段可以改變骨的微細結(jié)構(gòu),而不能改變骨腱結(jié)合部的組織變化,骨的重建和恢復能力要強于骨腱結(jié)合部;②Micro-CT測定法可以更好描述纖維軟骨帶厚度的假設不成立。技術(shù)由于技術(shù)特點的限制,不能準確的計算骨腱結(jié)合部纖維軟骨帶的面積。
[Abstract]:Patellar patellar tendon joint injury is a common sports injury and a hot issue in sports science.Animal experiment is an important part in the study of patellar patellar tendon joint injury mechanism and treatment.On the basis of the animal model of Nakama cyclic loading and the previous stage experiment, the effect of subthreshold intensity exercise on the injury of bone tendon joint was studied, and the feasibility of calculating the thickness of fibrous chondrocytes by Micro-CT technique was discussed.Micro-CT technique was used to calculate the changes of bone mass and trabeculae in the experimental group and the control group.Sixteen adult female New Zealand white rabbits were randomly divided into 4 weeks cyclic loading group and 8 week cyclic loading group. The left hind limb was tested and the right hind limb was used as blank control.In the first four weeks, the two groups received the muscle contraction stimulation with 7.17N/S intensity 3 times a week, 2 hours once for 24 hours.Four weeks after the experiment, the cyclic loading group stopped the experiment and the 8-week cyclic load group changed the strength to 4.2 N / S, the experiment time was unchanged, and the sample was collected. The tissue was reconstructed by Micro-CT first, then fixed and decalcified.Tissue sections were made by dehydration, paraffin embedding, sections and HE staining, and histologic analysis was performed by image acquisition system.Results: compared with the control group, the 8-week cyclic loading group and the 4-week cyclic load group showed glass-like changes and fibrous disorder in the patellar tendon, the tidal line appeared "high tide", blurred or even disappeared, and the fibrous cartilage band thickened.The results showed that there was no significant difference between 8-week cyclic load group and 4-week cyclic load group in the thickness of fibrous chondrocytes by HE staining. There was no correlation between the results of micro-CT and HE staining.The results of bone mass and trabeculae calculated by Micro-CT showed that there was no significant difference between the experimental group and the control group.Conclusion the intervention of subthreshold stress training and rest on patellar tendon junction injury can change the fine structure of bone, but not the tissue change of tendon junction. The ability of bone reconstruction and recovery is stronger than that of tendon junction.The hypothesis that the thickness of fibrochondrocytes can be better described by 2Micro-CT method is not true.Due to the limitation of technical characteristics, the area of fibrous cartilage band of bone tendon junction can not be calculated accurately.
