【摘要】：目的:探討乳腺M(fèi)RI檢查對乳腺X線攝影中結(jié)構(gòu)扭曲表現(xiàn)的診斷價值。方法:搜集2014年1月1日-2015年3月10日本院行乳腺X線攝影檢查患者的相關(guān)臨床、影像及病理資料,連續(xù)性納入乳腺X線攝影診斷為腺體結(jié)構(gòu)扭曲且術(shù)前行乳腺M(fèi)RI檢查的患者,排除病變部位有既往外傷或手術(shù)史的患者。最終入組32例病例,其中22例單純結(jié)構(gòu)扭曲,10例合并微鈣化、局灶不對稱致密或腫塊等征象。分別分析X線攝影腺體結(jié)構(gòu)扭曲伴有其他征象者、單純性結(jié)構(gòu)扭曲者的陽性預(yù)測值(PPV)及進(jìn)一步MRI檢查對診斷效能的影響。結(jié)果:乳腺X線攝影表現(xiàn)為腺體結(jié)構(gòu)扭曲且有伴隨征象者,PPV為66.7%(8/12),進(jìn)一步MRI檢查減少30%的假陽性診斷;表現(xiàn)為單純結(jié)構(gòu)扭曲者,PPV為9.1%(2/22),進(jìn)一步MRI檢查減少86.4%的假陽性診斷;未發(fā)現(xiàn)MRI漏診病例。結(jié)論:對于乳腺X線攝影表現(xiàn)為單純腺體結(jié)構(gòu)扭曲者,進(jìn)一步MRI檢查能夠顯著提高PPV。
[Abstract]:Objective: to evaluate the diagnostic value of mammary MRI in structural distortion in mammography.Methods: the clinical, imaging and pathological data of patients undergoing mammography from January 1, 2014 to March 10, 2015 in our hospital were collected. The patients diagnosed by continuous mammography as distorted glandular structure and MRI before mammography were included in this study.Patients with previous trauma or surgical history were excluded from the lesion.Finally, 32 cases were included in the study, including 22 cases with simple structural distortion and 10 cases with microcalcification, focal asymmetry or mass.The positive predictive value (PPVs) and the effect of further MRI examination on the diagnostic efficacy were analyzed in patients with structural distortion of glandular structure and other signs in X-ray radiographs, and in patients with simple structural distortion (PPVs) and in patients with simple structural distortions (PPVs).No missed diagnosis of MRI was found.Conclusion: if mammography shows simple glandular structure distortion, further MRI examination can significantly improve PPVs.
