【摘要】：目的研究乳腺癌兩種不同固定技術(shù)對調(diào)強放療精確度的影響。方法收集本院80例施行VMAT技術(shù)放療乳腺癌病例。40例使用個體化真空固定墊及形狀記憶熱塑網(wǎng)膜固定技術(shù)設(shè)為A組;其余40例則使用個體化真空墊及體表標(biāo)線固定技術(shù)設(shè)為B組。A、B組首次放療前均進行CBCT掃描,并記錄A、B兩組首次線性、旋轉(zhuǎn)擺位誤差,當(dāng)線性誤差大于5 mm,旋轉(zhuǎn)誤差大于20時進行校正后治療,記錄并分析其通過率。結(jié)果 1A組X、Y、Z、Rx、Ry、Rz軸方向上首次擺位誤差為:(2.18±1.78)mm、(3.01±3.10)mm、(2.64±3.53)mm、(1.07±0.88)0、(1.17±0.88)0、(0.71±0.65)0;B組X、Y、Z、Rx、Ry、Rz軸方向上首次擺位誤差為:(4.00±3.32)mm、(3.67±2.77)mm、(4.3±3.4)mm、(1.37±1.19)0、(0.82±0.83)0、(0.79±0.65)0,A組X、Z軸的線性擺位誤差高于B組,具有統(tǒng)計學(xué)意義(P0.05);2A、B兩組X、Y、Z軸方向線性擺位誤差≤3 mm的通過率分別為:30(75%)、26(65%)、29(72.5%);18(45%)、21(52.0%)、16(40%);A、B兩組Rx、Ry、Rz方向上旋轉(zhuǎn)擺位誤差≤20的通過率分別為:35(85%)、33(82.5%)、38(95%);26(65.5%)、36(90%)、36(90%)。A組X、Z、Rx方向的擺位誤差通過率高于B組,有顯著性統(tǒng)計學(xué)意義(P0.05)。3A、B兩組X、Y、Z軸方向線性擺位誤差≥5 mm的通過率分別為:37(92.5%)、30(75%)、37(92.5%);29(72.5%)、28(70%)、22(55%)。A、B兩組Rx、Ry、Rz方向上旋轉(zhuǎn)擺位誤差≥30的通過率分別為:38(95%)、38(95%)、40(100%);36(90%)、39(97.5%)、40(100%)。A組X、Z方向的擺位誤差通過率高于B組,有顯著性統(tǒng)計學(xué)意義(P0.05)。大于30的旋轉(zhuǎn)誤差各軸位上均無顯著性差異(P0.05)。在使用形狀記憶熱塑網(wǎng)膜時Z、Y、Z軸CTV-PTV外放間距分別為:6.70 mm、9.70 mm、9.07 mm;不使用形狀記憶熱塑網(wǎng)膜時Z、Y、Z軸CTV-PTV外放間距增加到:12.32 mm、11.11 mm、13.13 mm。結(jié)論乳腺癌調(diào)強放療使用個體化真空固定墊及形狀記憶熱塑網(wǎng)膜固定技術(shù)能顯著減少擺位誤差,提高首次擺位通過率,縮小CTV-PTV外擴范圍,有利于提高放療擺位的精確度。
[Abstract]:Objective to study the effect of two different fixation techniques on the accuracy of IMRT.Methods 80 patients with breast cancer received VMAT radiotherapy. 40 patients were divided into group A with individualized vacuum fixator and thermoplastic omentum fixation with shape memory.The other 40 cases were performed CBCT scan before the first radiotherapy in group B, and recorded the first linear and rotational pendulum errors of group A and B by using the technique of individualized vacuum pad and body surface marking.When the linear error is greater than 5 mm and the rotation error is greater than 20:00, the pass rate is recorded and analyzed.緇撴灉 1A緇刋,Y,Z,Rx,Ry,Rz杞存柟鍚戜笂棣栨鎽嗕綅璇樊涓，
