【摘要】：目的探討低注射速率、低劑量對(duì)比劑3D-DSA腦血管造影的的可行性。方法連續(xù)收集疑似動(dòng)脈瘤患者51例行3D-DSA腦血管造影,選擇DynaCT 5sDSA成像,均采用非離子型對(duì)比劑碘普羅胺注射液(370mgI/100ml),導(dǎo)管尖端均位于平樞椎水平,按對(duì)比劑注射速率隨機(jī)分為A組(1.5ml/s,n=18)、B組(2.0ml/s,n=18)、C組(3.0ml/s,n=15)。計(jì)算C臂錐形束CT軸位圖像頸內(nèi)動(dòng)脈巖段(C2段)和眼段(C6段)、大腦中動(dòng)脈水平段(M1段)、大腦前動(dòng)脈水平段(A1段)的噪聲、SNR及CNR,由2名醫(yī)師對(duì)連續(xù)減影圖像、VR及MIP重組圖像的圖像質(zhì)量進(jìn)行評(píng)分后行統(tǒng)計(jì)學(xué)分析。結(jié)果 B、C組與A組噪聲的差異均無統(tǒng)計(jì)學(xué)意義(P均0.05)。A組與B組比較,M1、A1段的SNR、CNR差異均有統(tǒng)計(jì)學(xué)意義(P均0.05),C2、C6段SNR、CNR差異均無統(tǒng)計(jì)學(xué)意義(P均0.05);A組與C組比較,C2、C6、M1及A1段的SNR、CNR差異均有統(tǒng)計(jì)學(xué)意義(P均0.05)。3組間大腦中動(dòng)脈、大腦前動(dòng)脈連續(xù)減影圖像及VR、MIP圖像的圖像質(zhì)量主觀評(píng)分差異均無統(tǒng)計(jì)學(xué)意義(P均0.05),均可充分顯示顱內(nèi)動(dòng)脈瘤情況。結(jié)論 3D-DSA腦血管造影中實(shí)施個(gè)性化低注射速率、低劑量對(duì)比劑注射方案可行,能夠降低患者碘攝入量及血管破裂出血風(fēng)險(xiǎn)。
[Abstract]:Objective to investigate the feasibility of low injection rate, low dose contrast agent 3D-DSA angiography.Methods 51 consecutive patients with suspected aneurysms underwent 3D-DSA angiography and DynaCT 5sDSA imaging was performed. All patients were treated with nonionic contrast agent iopramide injection 370mgI / 100ml, and the tip of the catheter was at the level of horizontal axis.According to the injection rate of contrast medium, group A was randomly divided into two groups: group A (1.5 ml / s) and group B (2. 0 ml / s ~ (18)) and group C (3. 0 ml / s ~ (-1)).C arm conical beam CT axial CT images of C ~ 2 segment of internal carotid artery), C _ 6 segment of ocular segment, M _ 1 segment of middle cerebral artery and A _ 1 segment of anterior cerebral artery were calculated.The image quality of the reconstructed image was evaluated and analyzed statistically.There were significant differences in CNR between two groups (P < 0.05).There was no significant difference in image quality between subtraction images of anterior cerebral artery and image quality of VRU MIP images (P < 0.05), and the intracranial aneurysms could be fully displayed.Conclusion the individualized low injection rate and low dose contrast agent injection regimen in 3D-DSA cerebral angiography are feasible and can reduce the iodine intake and the risk of blood vessel rupture.
