遞增循環(huán)載荷量致骨腱結(jié)合部過(guò)度使用損傷的發(fā)病機(jī)制研究

【摘要】:目的:通過(guò)遞增循環(huán)載荷量致過(guò)度使用損傷發(fā)生,在此過(guò)程從形態(tài)結(jié)構(gòu)、生長(zhǎng)因子、急性期炎性蛋白及細(xì)胞外基質(zhì)表達(dá)的變化,探索髕骨-臏腱結(jié)合部(Patella-Patella Tendon Junction, PPTJ)過(guò)度使用損傷的發(fā)病機(jī)制。 方法:45只成年雌性新西蘭大白兔,按循環(huán)載荷量不同隨機(jī)分成2h、4h、6h、12h和24h組。完成相應(yīng)的循環(huán)載荷量后,處死、取材、常規(guī)組織學(xué)處理。行HE、Safrain0染色以及CRP、生長(zhǎng)因子和細(xì)胞外基質(zhì)的免疫組化染色。MetaMorph Premier Offline對(duì)各組肌腱厚度、細(xì)胞密度、纖維軟骨帶厚度、膠原排列、粘多糖蛋白面積指標(biāo)組織形態(tài)計(jì)量。免疫組化的評(píng)定包括標(biāo)本陽(yáng)性染色率及陽(yáng)性染色細(xì)胞密度。SAS6.1.2進(jìn)行統(tǒng)計(jì)分析處理,顯著性水平為P0.05。 結(jié)果:與C相比:6h組細(xì)胞密度顯著增加(p0.01),12h、24h組纖維軟骨帶顯著增寬(p0.01),4h、6h、12h和24h組膠原排列顯著紊亂(p0.01);24h組肌腱厚度顯著低于12h組(p0.05);4h組b-FGF陽(yáng)性染色細(xì)胞密度顯著高于其它各組(p0.05);C組CTGF陽(yáng)性染色細(xì)胞密度顯著低于其它各組(p0.05);2h組TGF-β2、PDGF-B陽(yáng)性染色細(xì)胞密度均顯著高于C、6h、12h和24h組(p0.05);隨循環(huán)載荷量增加:VEGF的陽(yáng)性染色率顯著增加(p0.05); BMP-7標(biāo)本陽(yáng)性染色呈現(xiàn)線性增加趨勢(shì)(p0.05); Tenascin-C的陽(yáng)性染色細(xì)胞密度呈顯著的二次多項(xiàng)式趨勢(shì)(p0.05)。與C組相比,2h和24h組Collagen-III陽(yáng)性染色細(xì)胞密度顯著升高(p0.05)。 結(jié)論:149759±7285N/kg體重的循環(huán)載荷量能成功建立PPTJ過(guò)度使用損傷動(dòng)物模型。在過(guò)度使用損傷發(fā)生過(guò)程中,PPTJ先后呈現(xiàn)出膠原排列紊亂-細(xì)胞密度增加-纖維軟骨帶增寬的病理變化過(guò)程。VEGF表達(dá)的增加使PPTJ血管增生,bFGF的降低使得PPTJ修復(fù)能力下降,TGF-β2表達(dá)減少降低肌腱的抗?fàn)坷芰?CTGF的持續(xù)高表達(dá)引起PPTJ膠原的交聯(lián),四者表達(dá)的變化可能促使過(guò)度使用損傷發(fā)生。Ⅲ型膠原表達(dá)增加加劇了PPTJ過(guò)度使用損傷發(fā)展,Tenascin-C表達(dá)的降低使得細(xì)胞外基質(zhì)的機(jī)械穩(wěn)定性降低,二者表達(dá)變化可能促進(jìn)過(guò)度使用損傷的發(fā)生。
[Abstract]:Aim: to change the expression of inflammatory proteins and extracellular matrix (ECM) in the process of overuse injury caused by increasing cyclic load, including morphological structure, growth factor, inflammatory protein and extracellular matrix. Objective: to explore the pathogenesis of patella-patella Tendon Junction (PPTJ) overuse injury in patellar patellar tendon junction. Methods Forty-five adult female New Zealand white rabbits were randomly divided into two groups according to their cyclic load. Routine histopathology. Hefa Safrain0 staining and immunohistochemical staining of CRP, growth factor and extracellular matrix .Meta Morph Premier Offline on tendon thickness, cell density, fibrochondral band thickness, collagen arrangement in each group. Tissue morphometry of mucopolysaccharide protein area index. Immunohistochemical evaluation included positive staining rate and positive staining cell density. SAS6.1.2 for statistical analysis, the significant level was P0.05. Results: compared with C group, the cell density of group C was significantly higher than that of group C for 12 h and 24 h. The proliferation of fibrochondrocytes was significantly wider in group P 0.01C for 4 h and in group C for 24 h. The thickness of tendon in group 24 h was significantly lower than that in group C (group C) was significantly lower than that in group C (24 h), and the density of b-FGF positive cells in group 24 h was significantly higher than that in group B (0.05 h). The cell density of CTGF positive staining in group C was significantly lower than that in group C (2 h), and the density of positive cells in group C was significantly higher than that in group C (12 h and 24 h), the positive staining rate of CTGF was significantly increased with the increase of cyclic load, and the positive staining rate of BMP-7 was significantly higher than that of group C (P 0.05). The positive cell density of TGF- 尾 2 + PDGF-B in group C was significantly higher than that in group C at 6 h and 24 h, and the positive staining rate of CTGF was significantly increased with the increase of cyclic load. The positive staining cell density of Tenascin-C showed a significant quadratic polynomial trend. Compared with group C, the density of Collagen-III positive staining cells in group C was significantly higher than that in group C (2 h and 24 h). Conclusion the PPTJ model of overuse injury can be successfully established by the cyclic load of 1: 149759 鹵7285N-1 / kg body weight. In the process of overuse injury, the disorder of collagenous arrangement, cell density increase and fibrochondral band widening can be seen successively in the process of overuse injury. The increased expression of VEGF decreased the repair ability of PPTJ and decreased the expression of TGF- 尾 2. The continued high expression of TGF- 尾 2 resulted in the cross-linking of PPTJ collagen. Increased expression of type 鈪,
