經橈動脈介入術后兩臂血壓測值的變化及影響因素

【摘要】:目的: 探討經橈動脈途徑介入術后血壓測值的變化及影響因素。 方法: 選取2011年2月至2011年10月于我院擇期擬行經右側橈動脈途徑冠狀動脈造影(CAG)和/或冠狀動脈介入治療(PCI)的患者154例,按造影結果分為正常組和病變組,收集患者一般資料(性別、年齡、身高、體重、臂圍等),分別于術前、術后1天、術后7天、術后90天以及術后≥180天同步測量患者的兩側肱動脈和橈動脈血壓值,分析比較術后血壓測值的變化及影響因素。 結果: 1、正常組術前左右側肱動脈收縮壓、舒張壓均無差異(SBP:122.3±17.6mmHg vs122.4±17.9mmHg;DBP:70.6±10.0mmHg vs71.0±10.1mmHg,P>0.05);病變組術前左右側肱動脈收縮壓、舒張壓無差異(SBP:118.4±12.7mmHg vs119.7±13.4mmHg;DBP:68.0±8.0mmHg vs68.6±8.4mmHg,P>0.05);正常組術前左右側橈動脈收縮壓、舒張壓均無差異(SBP:119.9±11.9mmHg vs120.8±12.1mmHg;DBP:69.5±7.5mmHg vs70.4±7.6mmHg,P>0.05);病變組術前左右側橈動脈收縮壓、舒張壓無差異(SBP:121.6±10.5mmHg vs122.5±10.1mmHg;DBP:70.8±6.2mmHg vs71.3±6.8mmHg,P>0.05)。 2、正常組術后1天右側肱動脈收縮壓、舒張壓低于左側(SBP:117.8±14.6mmHg vs124.9±14.5mmHg;DBP:67.4±8.1mmHg vs71.5±7.9mmHg,P<0.05),術后7天右側肱動脈收縮壓、舒張壓仍低于左側(SBP:117.4±10.4mmHg vs121.6±11.1mmHg;DBP:67.1±5.8mmHg vs70.8±6.7mmHg,P<0.05);病變組術后1天右側肱動脈收縮壓、舒張壓低于左側(SBP:116.5±14.1mmHg vs123.6±13.7mmHg;DBP:66.0±8.9mmHg vs72.7±8.8mmHg,P<0.05),術后7天右側肱動脈收縮壓、舒張壓仍低于左側(SBP:113.0±8.9mmHgvs120.6±9.0mmHg;DBP:67.2±5.5mmHg vs71.7±6.3mmHg,P<0.05);正常組和病變組術后90天及術后≥180天兩側肱動脈收縮壓和舒張壓均無差異(P>0.05),無統(tǒng)計學意義。 3、正常組術后1天、術后7天左右側橈動脈收縮壓分別為(119.9±12.6mmHgvs113.0±11.7mmHg,P<0.05)、(118.3±8.7mmHg vs114.1±8.5mmHg,P<0.05),有統(tǒng)計學差異,舒張壓分別為(71.8±8.6mmHg vs65.6±8.7mmHg,P<0.05)、(71.6±6.2mmHg vs67.1±5.9mmHg,P<0.05),同樣有統(tǒng)計學差異;病變組左右側橈動脈收縮壓術后1天(125.4±15.1mmHg vs115.6±14.0mmHg,P<0.05)、術后7天(120.9±11.5mmHg vs114.8±10.7mmHg,P<0.05)均有顯著差異,舒張壓分別為(72.6±9.2mmHg vs67.0±7.7mmHg,P<0.05)、(71.6±7.4mmHg vs68.3±7.0mmHg,P<0.05),有統(tǒng)計學差異。正常組和病變組術后90天、術后≥180天左右側橈動脈收縮壓和舒張壓均無差異(P>0.05)。 結論: 經橈動脈介入術后,術側肱動脈和橈動脈收縮壓、舒張壓均低于對側,尤其在冠狀動脈造影證實有病變組中更明顯,但這種差異隨著時間的延長而逐漸減小。
[Abstract]:Objective:. To investigate the changes of blood pressure after transradial approach and its influencing factors. Methods:. 154 patients with CAG and / or PCI were selected from February 2011 to October 2011 in our hospital. They were divided into normal group and pathological group according to the results of angiography. The blood pressure of bilateral brachial and radial arteries were measured simultaneously before operation, 1 day after operation, 7 days after operation, 90 days after operation and more than 180 days after operation (gender, age, height, weight, arm circumference, etc.). The changes and influencing factors of blood pressure after operation were analyzed and compared. Results:. 1. The systolic and diastolic blood pressure of the left and right brachial artery in the normal group were not different before operation, SBP: 122.3 鹵17.6mmHg vs122.4 鹵17.9 mmHg vs122.4 鹵17.9 mmHg DBP: DBP: 70.6 鹵10.0mmHg vs71.0 鹵10.1mmHg (P > 0.05), SBP: 118.4 鹵12.7mmHg vs119.7 鹵13.4mmHg vs68.6 鹵8.4mmHg vs68.6 鹵8.4mmHgg P > 0.05g / g, the systolic blood pressure of the left and right brachial artery was not different before operation. There was no significant difference in diastolic blood pressure (SBP: 119.9 鹵11.9mmHg vs120.8 鹵12.1 mmHg vs120.8 鹵12.1mmHgU DBP: 69.5 鹵7.5mmHg vs70.4 鹵7.6mm Hgg P > 0.05), but there was no difference in systolic pressure between left and right radial artery (SBP: 121.6 鹵10.5mmHg vs122.5 鹵10.1mmHg vs71.3 鹵6.8mmHg vs71.3 > 0.05mmg P > 0.05mmHg). 2. The systolic blood pressure of right brachial artery in normal group was lower than that in left SBP: 117.8 鹵14.6mmHg vs124.9 鹵14.5 mmHg vs124.9 鹵14.5 mmHg DBP: 67.4 鹵8.1mmHg vs71.5 鹵7.9mmHgN P < 0.05 on the 1st day after operation. The systolic blood pressure of right brachial artery was still lower than that of left SBP117.4 鹵10.4mmHg vs121.6 鹵11.1 mmHgHg vs70.8 鹵6.7mmHgG (P < 0.05), and the systolic blood pressure of right brachial artery was still lower than that of left SBP (117.4 鹵10.4mmHg vs121.6 鹵11.1 mmHg vs70.8 鹵6.7mmHgG) on the 1st day after operation. The diastolic pressure was lower than that of the left SBP: 116.5 鹵14.1mmHg vs123.6 鹵13.7 mmHg DBP: 66.0 鹵8.9mmHg vs72.7 鹵8.8mm Hg (P < 0.05). The systolic blood pressure of the right brachial artery was still lower than that of the left SBP: 113.0 鹵8.9mmHgvs120.6 鹵9.0mmHgvs120.6 鹵9.0mmHgvs120.6 鹵5.5mmHg vs71.7 鹵6.3mmHg (P < 0.05). There was no significant difference in systolic and diastolic blood pressure of the brachial artery between the normal group and the pathological group (P > 0.05). 3The systolic blood pressure of left and right radial artery in normal group was 119.9 鹵12.6mmHgvs113.0 鹵11.7mmHgV (P < 0.05 鹵8.7mmHg vs114.1 鹵8.5mmHgU P < 0.05). The diastolic blood pressure was 71.8 鹵8.6mmHg vs65.6 鹵8.7mmHgg (P < 0.071.6 鹵6.2mmHg vs67.1 鹵5.9mmHgg), respectively. There were significant differences in left and right radial artery systolic blood pressure (125.4 鹵15.1mmHg vs115.6 鹵14.0mmHgN P < 0.05) on the 1st day after operation and 120.9 鹵11.5mmHg vs114.8 鹵10.7mmHgGG (P < 0.05) on the 7th day after operation. The diastolic blood pressure was 72.6 鹵9.2mmHg vs67.0 鹵7.7mmHg vs67.0 < 0.05hgg P < 0.05hgg P < 0.05, respectively. There was statistical difference between the normal group and the pathological group on the 90th day after operation, there was significant difference between the normal group and the pathological group on the 90th day after operation, the diastolic blood pressure was 72.6 鹵9.2mmHg vs67.0 鹵7.7mmHg vs67.0 < 0.05mmHg < 0.05mmHg (P < 0.05).
