【摘要】：目的比較X線正、側(cè)、斜位及CT掃描對成人橈骨頭細微骨折的檢出率。方法回顧性分析26例橈骨頭細微骨折的X線和CT圖像。分別統(tǒng)計肘關(guān)節(jié)正、側(cè)、斜位和CT掃描對橈骨頭細微骨折的檢出率并進行比較。結(jié)果 26例橈骨頭細微骨折首次檢出例數(shù):正位0例、側(cè)位0例、斜位19例(73.1%),CT 23例(88.5%)。骨折檢出率斜位和CT均優(yōu)于正、側(cè)位(χ2=29.94,P㩳0.05;χ2=41.24,P㩳0.05),斜位與CT檢出率差異無統(tǒng)計學意義(χ2=1.981,P0.05);2~4周后復查骨折檢出例數(shù):正位18例(69.2%),側(cè)位16例(61.5%)、斜位26例(100%),CT 26例(100%)。斜位與CT檢出率均優(yōu)于正、側(cè)位(P㩳0.05)。復查后正、側(cè)位、斜位檢出率均高于首次檢出率(P㩳0.05),復查前、后CT檢出率差異無統(tǒng)計學意義(P0.05)。結(jié)論斜位投照和CT檢查對橈骨頭細微骨折具有極高的檢出率,2~4周后復查是提高常規(guī)肘關(guān)節(jié)正、側(cè)位及斜位片檢出率的可靠方法,斜位片一定程度上可替代CT作為診斷橈骨頭細微骨折的首選方法。
[Abstract]:Objective to compare the positive, lateral, oblique and CT scans in the detection of minor fractures of the radial head in adults. Methods the X-ray and CT images of 26 cases of minor fractures of the radial head were retrospectively analyzed. Results 26 cases of radial head minor fractures were detected for the first time: positive position 0 cases, lateral position 0 cases, oblique position 19 cases 73.1% CT 23 cases. The detection rate of fracture in oblique position and CT scan was better than that in CT. Lateral position (蠂 2, 29.94 P? 0. 05; 蠂 2: 41.24? There was no significant difference between the positive rate of oblique position and that of CT (蠂 ~ (2) 1. 981 ~ (1) P 0.05 ~ 2 ~ 2 weeks later): 18 cases of positive position (69.2%), 16 cases of lateral position (61.5%), 26 cases of oblique position (26 cases) and 26 cases (26 cases) of oblique position were superior to positive position (P < 0.05), and the detection rate of lateral position was better than that of lateral position (P < 0.05). There was no significant difference in the detection rate between oblique position and CT (蠂 ~ (2)). The positive, lateral and oblique detection rates were higher than those in the first positive, lateral and oblique position after reexamination. There was no significant difference in the detection rate of posterior CT before and after reexamination (P 0.05). Conclusion oblique radiography and CT examination are reliable methods to improve the positive, lateral and oblique radiographs of the elbow joint after 4 weeks. Oblique radiography can replace CT as the first choice for the diagnosis of radial head fracture.
