【摘要】：目的:探討低管電壓、低對(duì)比劑碘量和iDose~4迭代重建技術(shù)(IR)組合在支氣管動(dòng)脈(BA)CT血管成像的應(yīng)用價(jià)值。方法:選取肺癌或咯血患者80例。隨機(jī)分為40例行常規(guī)劑量即120kV、200mAs行CTA檢查,注射對(duì)比劑80ml,采用濾波反投影重建法(FBP)進(jìn)行后處理,標(biāo)記為A組。40例患者行低劑量為80kV、250mAs進(jìn)行CTA檢查,注射對(duì)比劑60ml,采用FBP進(jìn)行重建者標(biāo)記為B組、采用iDose~46級(jí)進(jìn)行重建者標(biāo)記為C組。記錄各組的噪聲、信噪比、對(duì)比度噪聲比和有效劑量,按5分評(píng)價(jià)總體圖像質(zhì)量,并對(duì)3組結(jié)果進(jìn)行比較。結(jié)果:患者80例均順利完成檢查。所有患者BA的CT值均高于300HU,A、C組MIP和VR支氣管動(dòng)脈重建圖像均能滿足診斷需要;C組mSv為1.89±0.35,A組mSv為4.40±0.69,有效劑量(ED)顯著降低(t=20.518,P0.001),輻射劑量降低約57%,總碘量下降39.2%,碘流率下降18.9%。使用iDose~46級(jí)重建的C組主觀評(píng)分與A組差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),SNR、CNR均無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。結(jié)論:低管電壓(80kV)+iDose~4+低對(duì)比劑碘量聯(lián)合應(yīng)用在CTA檢查中支氣管動(dòng)脈獲得滿足診斷要求的圖像。
[Abstract]:Objective: to evaluate the application value of low tube voltage, low contrast agent iodine and iDose~4 iterative reconstruction technique in CT angiography of bronchial artery. Methods: 80 patients with lung cancer or hemoptysis were randomly divided into 40 routine doses (120 kV / 200mAs) for CTA examination. A contrast agent of 80 ml was injected and treated with filter backprojection reconstruction (FBP). Group A, 40 patients, underwent CTA examination with low dose of 80kV, 250mAs, 60 ml of contrast agent was injected, and group B was labeled as group B for reconstruction by FBP. Group C was marked by iDose~46 level reconstruction. The noise, signal-to-noise ratio, contrast noise ratio and effective dose of each group were recorded, and the overall image quality was evaluated according to 5 points. Results: 80 patients completed the examination successfully. The CT value of BA in all the patients was higher than that of MIP and VR bronchoarterial reconstruction images in 300HUAZC group. The mSv of group C was 1.89 鹵0.35 and the mSv of group A was 4.40 鹵0.69, which was effective. The dose of ED2 was significantly lower than that of group A, the radiation dose was decreased by about 57, the total iodine content was decreased by 39.2 and the iodine flow rate was decreased by 18.9.The subjective score of group C with iDose~46 grade reconstruction was not significantly different from that of group A. there was no significant difference between group C and group A (P 0.05). Conclusion: low tube voltage is 80kV) iDose~4. Combined use of low contrast iodine in CTA to obtain images of bronchial arteries that meet the diagnostic requirements.
