【摘要】：目的探討腎上腺區(qū)副脾的CT特征。方法回顧性分析經(jīng)手術(shù)病理證實(shí)的5例腎上腺區(qū)副脾術(shù)前CT資料。結(jié)果病灶均位于左側(cè)腎上腺區(qū),單發(fā),邊界清晰,4例呈類圓形,1例橢圓形,最大徑1.1~3.0 cm之間,平均(2.1±0.7)cm。CT平掃5例病灶密度均勻,與同層面的脾臟密度相近,均無囊變、壞死、出血及鈣化,CT值41.9~55.3 HU,平均(47.7±5.4)HU;增強(qiáng)動(dòng)脈期均明顯不均勻或斑馬紋樣強(qiáng)化,CT值78.6~141.1 HU,平均(102.6±25.8)HU,靜脈期2例進(jìn)一步強(qiáng)化,3例強(qiáng)化程度略減低,強(qiáng)化程度均趨于均勻,CT值68.7~129.7 HU,平均(104.6±22.9)HU。5例病灶3期CT值與同層面脾臟的CT值相仿,5例中,病灶由脾動(dòng)脈分支供血4例,合并其他部位的副脾2例。結(jié)論腎上腺區(qū)副脾CT特征為平掃密度及強(qiáng)化程度均相近,多由脾動(dòng)脈分支供血,易合并其他部位的副脾,這些特征有助于腎上腺區(qū)副脾的診斷。
[Abstract]:Objective to investigate the CT features of adrenal accessory spleen. Methods CT findings of 5 cases of adrenal accessory spleen confirmed by surgery and pathology were retrospectively analyzed. The maximum diameter was between 1.1 and 3.0 cm, the mean value was 2.1 鹵0.7 cm. Ct plain scan showed that the density of the lesions was uniform, similar to that of the spleen on the same plane, and no cystic degeneration or necrosis was found in all cases. The CT value of haemorrhage and calcification was 41.9 鹵55.3 HUU (mean 47.7 鹵5.4 HUU), the enhancement of arterial phase was obviously uneven or the CT value of zebra pattern enhancement was 78.6 鹵25.8 HUU (mean 102.6 鹵25.8 HUU), and the enhancement degree of 2 cases with further enhancement in venous phase was slightly decreased. The enhancement degree tended to homogenize with the CT value of 68.7 ~ 129.7 HU.The mean CT value of the lesions in the 5 cases was 104.6 鹵22.9HU, which was similar to the CT value of the same plane spleen in 5 cases, and the focus was supplied by the branches of the splenic artery in 4 cases. ConclusionThe CT features of adrenal accessory spleen are similar in density and enhancement, most of them are supplied by splenic artery, and they are easy to be combined with the accessory spleen of other parts. These characteristics are helpful for the diagnosis of adrenal accessory spleen.
