雙源CT降低時間分辨率對腦缺血疾病灌注圖像的影響

【摘要】:目的 探討基于最大斜率法(maximal slope, MS)數(shù)學模型,利用不同方法降低時間分辨率,評價其對腦缺血疾病CT灌注(CT perfusion imaging, CTPI)圖像質量及灌注參數(shù)的影響,尋求在保證診療要求的前提下降低CT灌注輻射劑量的新思路。 方法 回顧性分析我院經臨床及影像學資料確診的29例腦缺血患者CT灌注圖像資料。對時間-密度曲線(Time-density curve, T-DC)分別利用全部圖像數(shù)據(jù)(原始組)、每兩層選取一層間隔處理圖像數(shù)據(jù)(實驗組1)及在保證關鍵圖像數(shù)據(jù)選取的前提下間隔選取圖像數(shù)據(jù)(實驗組2)三種方法進行處理,然后對灌注偽彩圖像進行主觀視覺評價,并在不同組別圖像中選取相同感興趣區(qū)對腦血流量、腦血容量、平均通過時間等參數(shù)進行測量,對所得灌注參數(shù)值進行統(tǒng)計學分析。 結果 1、時間分辨率的改變對灌注偽彩圖像主觀視覺評分有一定影響,主觀評分較原始組均有所下降,實驗組1較實驗組2下降更為明顯,其中對MTT、CBF圖像主觀評分影響較為明顯,但不影響病灶檢出。 2、(1)實驗組1與原始組比較,CBF、CBV變化具有統(tǒng)計學意義(P0.05),而MTT變化則無統(tǒng)計學意義(P0.05)。(2)實驗組2與原始組比較,CBF變化具有統(tǒng)計學意義(P0.05),而CBV、MTT變化則無統(tǒng)計學意義(P0.05)。(3)將缺血側/健側灌注參數(shù)的比值作為研究對象,實驗組1與原始組、實驗組2與原始組分別進行比較,各相對參數(shù)值變化均無統(tǒng)計學意義(P0.05)。 結論 在一定范圍內降低時間分辨率,灌注偽彩圖像質量下降,但不影響病灶的檢出;但若對灌注參數(shù)值進行研究,建議使用較高時間分辨率對灌注圖像進行采集;時間分辨率的降低對灌注參數(shù)相對值的影響比對灌注參數(shù)絕對值的影響要;在保證關鍵圖像數(shù)據(jù)前提下改變時間分辨率較機械間隔選取圖像數(shù)據(jù)更能保證灌注參數(shù)的準確性。
[Abstract]:Purpose Based on the maximum slope method and maximum slope (MS) mathematical model, different methods are used to reduce the temporal resolution. Objective: to evaluate the effect of CT perfusion CT perfusion imaging (CTPI) on the quality and perfusion parameters of cerebral ischemia. To find a new way to reduce the dose of CT perfusion radiation on the premise of ensuring diagnosis and treatment. Method Ct perfusion imaging data of 29 patients with cerebral ischemia confirmed by clinical and imaging data in our hospital were retrospectively analyzed. Time-density curve of time-density curve were analyzed. T-DC) uses all the image data (original group) respectively. There are three methods for processing image data (experimental group 1) and for processing image data (experiment group 2) at interval under the premise of ensuring the selection of key image data. Then the perfusion pseudochromatic images were evaluated by subjective vision, and the same regions of interest were selected from different groups of images to measure the parameters of cerebral blood flow, cerebral blood volume, mean transit time and so on. The perfusion parameters were statistically analyzed. Results 1. The change of time resolution had a certain effect on the subjective visual score of perfusion pseudochromogram, the subjective score was lower than that of the original group, and the decrease of experimental group 1 was more obvious than that of experimental group 2, in which MTT was more obvious. The subjective score of CBF images was more significant, but it did not affect the detection of lesions. (2) compared with the original group, the change of CBV in experimental group 1 was statistically significant (P 0.05). But the change of MTT was not statistically significant (P 0.05) the change of MTT in experimental group 2 was statistically significant compared with the original group (P 0.05), but CBV. The ratio of perfusion parameters of ischemic side to normal side was taken as the research object. The experimental group 1 was compared with the original group, and the experimental group 2 was compared with the original group. No statistical significance was found in the changes of relative parameters (P 0.05). Conclusion In a certain range, the time resolution was reduced, the quality of perfusion pseudochromogram was decreased, but the detection of lesions was not affected. However, if the perfusion parameters are studied, it is suggested to use higher temporal resolution to collect the perfusion image. The decrease of time resolution has less effect on the relative value of perfusion parameters than on the absolute values of perfusion parameters. The accuracy of perfusion parameters can be ensured by changing the time resolution than selecting the image data at mechanical intervals under the premise of ensuring the key image data.
