雙源CT降低時(shí)間分辨率對(duì)腦缺血疾病灌注圖像的影響

【摘要】:目的 探討基于最大斜率法(maximal slope, MS)數(shù)學(xué)模型,利用不同方法降低時(shí)間分辨率,評(píng)價(jià)其對(duì)腦缺血疾病CT灌注(CT perfusion imaging, CTPI)圖像質(zhì)量及灌注參數(shù)的影響,尋求在保證診療要求的前提下降低CT灌注輻射劑量的新思路。 方法 回顧性分析我院經(jīng)臨床及影像學(xué)資料確診的29例腦缺血患者CT灌注圖像資料。對(duì)時(shí)間-密度曲線(Time-density curve, T-DC)分別利用全部圖像數(shù)據(jù)(原始組)、每兩層選取一層間隔處理圖像數(shù)據(jù)(實(shí)驗(yàn)組1)及在保證關(guān)鍵圖像數(shù)據(jù)選取的前提下間隔選取圖像數(shù)據(jù)(實(shí)驗(yàn)組2)三種方法進(jìn)行處理,然后對(duì)灌注偽彩圖像進(jìn)行主觀視覺評(píng)價(jià),并在不同組別圖像中選取相同感興趣區(qū)對(duì)腦血流量、腦血容量、平均通過時(shí)間等參數(shù)進(jìn)行測(cè)量,對(duì)所得灌注參數(shù)值進(jìn)行統(tǒng)計(jì)學(xué)分析。 結(jié)果 1、時(shí)間分辨率的改變對(duì)灌注偽彩圖像主觀視覺評(píng)分有一定影響,主觀評(píng)分較原始組均有所下降,實(shí)驗(yàn)組1較實(shí)驗(yàn)組2下降更為明顯,其中對(duì)MTT、CBF圖像主觀評(píng)分影響較為明顯,但不影響病灶檢出。 2、(1)實(shí)驗(yàn)組1與原始組比較,CBF、CBV變化具有統(tǒng)計(jì)學(xué)意義(P0.05),而MTT變化則無統(tǒng)計(jì)學(xué)意義(P0.05)。(2)實(shí)驗(yàn)組2與原始組比較,CBF變化具有統(tǒng)計(jì)學(xué)意義(P0.05),而CBV、MTT變化則無統(tǒng)計(jì)學(xué)意義(P0.05)。(3)將缺血側(cè)/健側(cè)灌注參數(shù)的比值作為研究對(duì)象,實(shí)驗(yàn)組1與原始組、實(shí)驗(yàn)組2與原始組分別進(jìn)行比較,各相對(duì)參數(shù)值變化均無統(tǒng)計(jì)學(xué)意義(P0.05)。 結(jié)論 在一定范圍內(nèi)降低時(shí)間分辨率,灌注偽彩圖像質(zhì)量下降,但不影響病灶的檢出;但若對(duì)灌注參數(shù)值進(jìn)行研究,建議使用較高時(shí)間分辨率對(duì)灌注圖像進(jìn)行采集;時(shí)間分辨率的降低對(duì)灌注參數(shù)相對(duì)值的影響比對(duì)灌注參數(shù)絕對(duì)值的影響要。辉诒WC關(guān)鍵圖像數(shù)據(jù)前提下改變時(shí)間分辨率較機(jī)械間隔選取圖像數(shù)據(jù)更能保證灌注參數(shù)的準(zhǔn)確性。
[Abstract]:Purpose Based on the maximum slope method and maximum slope (MS) mathematical model, different methods are used to reduce the temporal resolution. Objective: to evaluate the effect of CT perfusion CT perfusion imaging (CTPI) on the quality and perfusion parameters of cerebral ischemia. To find a new way to reduce the dose of CT perfusion radiation on the premise of ensuring diagnosis and treatment. Method Ct perfusion imaging data of 29 patients with cerebral ischemia confirmed by clinical and imaging data in our hospital were retrospectively analyzed. Time-density curve of time-density curve were analyzed. T-DC) uses all the image data (original group) respectively. There are three methods for processing image data (experimental group 1) and for processing image data (experiment group 2) at interval under the premise of ensuring the selection of key image data. Then the perfusion pseudochromatic images were evaluated by subjective vision, and the same regions of interest were selected from different groups of images to measure the parameters of cerebral blood flow, cerebral blood volume, mean transit time and so on. The perfusion parameters were statistically analyzed. Results 1. The change of time resolution had a certain effect on the subjective visual score of perfusion pseudochromogram, the subjective score was lower than that of the original group, and the decrease of experimental group 1 was more obvious than that of experimental group 2, in which MTT was more obvious. The subjective score of CBF images was more significant, but it did not affect the detection of lesions. (2) compared with the original group, the change of CBV in experimental group 1 was statistically significant (P 0.05). But the change of MTT was not statistically significant (P 0.05) the change of MTT in experimental group 2 was statistically significant compared with the original group (P 0.05), but CBV. The ratio of perfusion parameters of ischemic side to normal side was taken as the research object. The experimental group 1 was compared with the original group, and the experimental group 2 was compared with the original group. No statistical significance was found in the changes of relative parameters (P 0.05). Conclusion In a certain range, the time resolution was reduced, the quality of perfusion pseudochromogram was decreased, but the detection of lesions was not affected. However, if the perfusion parameters are studied, it is suggested to use higher temporal resolution to collect the perfusion image. The decrease of time resolution has less effect on the relative value of perfusion parameters than on the absolute values of perfusion parameters. The accuracy of perfusion parameters can be ensured by changing the time resolution than selecting the image data at mechanical intervals under the premise of ensuring the key image data.
