【摘要】：目的災(zāi)難救治現(xiàn)場,于野戰(zhàn)綜合手術(shù)救治方艙中對老年性骨質(zhì)疏松性椎體壓縮性骨折的受災(zāi)患者行經(jīng)皮椎體后凸成形術(shù)(PKP)的近期臨床療效分析。方法回顧性分析2013年4月—2016年8月由該院參與救治的老年性骨質(zhì)疏松性椎體壓縮性骨折患者43例(51椎),患者傷后立即在該院研制的野戰(zhàn)綜合手術(shù)救治方艙中行PKP手術(shù),觀察患者平均手術(shù)時間,比較手術(shù)前后痛覺視覺模擬評分(VAS)的變化和側(cè)位X線平片上相應(yīng)節(jié)段Cobb角的變化及椎體前、中、后緣高度的變化。結(jié)果本組43例患者平均手術(shù)時間(52±15)min,術(shù)后VAS疼痛評分較術(shù)前明顯降低(P0.01),術(shù)后傷椎相應(yīng)節(jié)段Cobb角較術(shù)前有顯著改變(P0.01),且傷椎椎體前、中緣高度較術(shù)前均由明顯恢復(fù)(P0.01),差異均有顯著統(tǒng)計學(xué)意義。結(jié)論在野戰(zhàn)綜合手術(shù)救治方艙中對骨質(zhì)疏松性壓縮性骨折患者行PKP治療是對老年患者緊急治療的十分有效的方法,能早期解決患者骨折所導(dǎo)致的劇烈疼痛,改善患者預(yù)后。
[Abstract]:The purpose of the disaster rescue scene, for the affected patients with compression fracture of osteoporotic vertebral surgery in a field where the shelter of percutaneous kyphoplasty (PKP) clinical analysis. Methods Retrospective analysis of April 2013 - August 2016 by the hospital in the treatment of senile osteoporotic vertebral compression fractures 43 patients (51 vertebrae), patients immediately after injury in the hospital development and comprehensive surgical treatment shelter field underwent PKP operation were observed, the average operation time, compared before and after surgery pain visual analogue scale (VAS) and the change of lateral X-ray changes in the corresponding segment of the Cobb angle and the vertebral body in. Results of the 43 patients, the average operation time (52 + 15) min, postoperative VAS pain score was significantly lower than that before operation (P0.01), postoperative vertebral segments have significantly changed corresponding compared with the preoperative Cobb angle (P0.01), and Vertebral anterior margin height, in compared with the preoperative by obvious recovery (P0.01), the difference was statistically significant. Conclusion in the comprehensive surgical field treatment to shelter the osteoporotic vertebral compression fractures were treated with PKP is an effective method for elderly patients in emergency treatment, can solve the severe pain in patients with early fracture lead, improve the prognosis of the patients.
