應(yīng)用多絲電離室實(shí)時(shí)監(jiān)測(cè)劑量和MLC的研究

【摘要】:調(diào)強(qiáng)放射治療(IMRT)通常利用加速器多葉光柵(MLC)形成多個(gè)不規(guī)則子野,其不同形狀大小和劑量的子野疊加導(dǎo)致了對(duì)腫瘤靶區(qū)的適形以及對(duì)束流的調(diào)制,從而實(shí)現(xiàn)患者的束流照射。臨床治療過(guò)程中如何確保MLC形成子野的精確性以及加速器束流照射劑量的準(zhǔn)確性則顯得至關(guān)重要,這也是實(shí)現(xiàn)精確放療有效性的重要保證。 本文分析探討了DAVID系統(tǒng)實(shí)施實(shí)時(shí)監(jiān)測(cè)的特點(diǎn)和各項(xiàng)性能,對(duì)其治療中監(jiān)測(cè)功能的可行性與實(shí)用性進(jìn)行了較為詳細(xì)地研究。多絲電離室(DAVID)作為一種新型的質(zhì)量保證設(shè)備,,可用于治療過(guò)程中實(shí)時(shí)監(jiān)測(cè)MLC形狀和束流劑量。通過(guò)研究DAVID對(duì)射線束流的影響,評(píng)價(jià)DAVID對(duì)射野的檢測(cè)精度和不同射野的可重復(fù)劑量,以及評(píng)估出DAVID對(duì)射野和劑量偏差的檢測(cè)能力,從而奠定了該系統(tǒng)在患者臨床應(yīng)用中實(shí)時(shí)監(jiān)測(cè)的理論基礎(chǔ)和實(shí)驗(yàn)依據(jù)。最后比較了患者IMRT治療中DAVID實(shí)時(shí)檢測(cè)的數(shù)據(jù),較為客觀地總結(jié)出臨床中DAVID實(shí)時(shí)監(jiān)測(cè)的有效性及其應(yīng)用價(jià)值。 測(cè)試結(jié)果表明,治療中DAVID系統(tǒng)能夠較好地實(shí)現(xiàn)MLC葉片位置和劑量精度的驗(yàn)證和監(jiān)測(cè);但也存在一定的局限性,需要在臨床實(shí)際應(yīng)用中加以注意和克服。
[Abstract]:Intensity modulated radiotherapy (IMRT) usually uses the accelerator multileaf grating (MLC) to form multiple irregular subfields. The superposition of the subfields with different shapes and doses leads to the conformability of the tumor target and the modulation of the beam current. How to ensure the accuracy of the formation of subfields of MLC and the accuracy of the dose of accelerator beam irradiation is very important in the clinical treatment. This is also an important guarantee for the effectiveness of accurate radiotherapy. In this paper, the characteristics and performance of real-time monitoring in DAVID system are analyzed and discussed. The feasibility and practicability of the monitoring function in the treatment were studied in detail. The multifilament ionization chamber DAVIDD is a new type of quality assurance equipment. It can be used to monitor the shape and dose of MLC in real time in the course of treatment. By studying the effect of DAVID on beam, the accuracy of detection of beam field by DAVID and the repeatable dose of different field are evaluated. The ability of DAVID to detect field and dose deviation was evaluated. Thus, the theoretical basis and experimental basis of real-time monitoring of DAVID in clinical application of patients were established. Finally, the data of real-time detection of DAVID in patients with IMRT were compared. The effectiveness and application value of real-time monitoring of DAVID in clinic were summarized objectively. The test results show that the DAVID system can better verify and monitor the accuracy of MLC leaf position and dose. However, there are some limitations which need to be paid attention to and overcome in clinical practice.
